Coagulation in chronic liver disease and the use of prothrombin complex concentrate for an emergent procedure: a case report and review of literature

Abstract: Synthetic dysfunction observed in cirrhosis results in altered production of procoagulants and anticoagulants that can lead to both bleeding and thrombotic events, respectively. In patients with decompensated cirrhosis, frequent hospital visits often require bedside procedures such as diagnostic paracenteses, thoracenteses and endoscopy. It can be difficult to determine at what coagulation threshold procedures can safely be performed. Currently, the most common therapies given pre-procedurally include fresh fr… Show more

“…suggests that PCC effectively reduce INR in patients with cirrhosis but it is still unclear whether this correlates with a reduction in bleeding. 57,58 Kwon et al retrospectively studied patients with cirrhosis who had received PCC, FFP, and rVFIIa prophylactically for a procedure. They found that those who had received PCC had significantly less pRBC and platelet transfusions and lower rates of volume overload compared to FFP.…”

“…The remainder of literature on PCC in cirrhotic patients is limited to retrospective studies and case reports, with studies on its use in acute hemorrhagic events being even more rare. The literature suggests that PCC effectively reduce INR in patients with cirrhosis but it is still unclear whether this correlates with a reduction in bleeding . Kwon et al retrospectively studied patients with cirrhosis who had received PCC, FFP, and rVFIIa prophylactically for a procedure.…”

“…Data on the benefit of prothrombin complex concentrates (PCC) in patients with cirrhosis are limited to retrospective studies and case reports[ 69 , 70 ]. PCC infusion was reported to significantly reduce INR in critically ill cirrhosis patients, with lower amount of blood transfusion requirements compared to FFP.…”
