Co-possession of phosphodiesterase type-5 inhibitors (PDE5-I) with nitrates

Abstract: Keeping in mind the limitations of observational studies, these results suggest that co-dispensing of nitrates and PDE5-Is is low. Compared to control patients, the proportion of nitrate co-possession was lowest for patients filling tadalafil. Tadalafil patients also had the lowest co-possessed proportion among the three PDE5-I cohorts. While the majority of co-possessed drug pairs were prescribed by different providers, the highest percentage of co-prescribing from the same physician was among cardiologists. … Show more

“…The percentage of nitrates co-prescribed with sildenafil, vardenafil and tadalafil was 4.1% (n=320), 1.07% (n=93) and 1.25% (n =80), respectively. This result was similar to the finding of Chang et al study that showed the co-prescribed percentages of 1.07%, 0.83% and 1.07% for sildenafil, vardenafil and tadalafil, respectively 37. Despite the overall result showing low incidence of contraindication, the need to double-check the intake of concomitant drugs is crucial for preventing severe hypotension 38…”

“…However, this study did not assess whether these participants used such medications only periodically and/or would be able to manage their medication to maintain the 24‐hour wash out period between the two agents. In the current situation, when PDE5i are prescribed by a physician, copossession rates of 2.1% [31] and 2.2% [32] have been reported in actual use studies. In addition, analysis of the manufacturer's postmarketing safety database shows that only about 1% of the almost 40,000 adverse events concerned comedication with nitrates.…”
