Co-morbidity of adult attention-deficit/hyperactivity disorder with focus on personality traits and related disorders in a tertiary referral center

Abstract: In a cohort of patients with AADHD referred to a single tertiary center co-morbidity with axis-I/II disorders was remarkably prevalent. In AADHD co-morbid mood, anxiety, and personality disorders as well as substance abuse/dependence is likely to be predictive of poor outcome.

“…Finally, allelic variation of TPH2 function appears to influence the risk of a variety of neuropsychiatric disorders such as ADHD and obsessive-compulsive disorder (OCD), clinical entities commonly associated with difficulties to control emotions and with a high co-morbidity of depression [39,40]. Transmission disequilibrium of potentially functional variants in the transcriptional control or in the coding region of TPH2 in ADHD [41 -43], and preferential transmission of a haplotype of TPH2 in early-onset OCD [44] were reported.…”

“…1 Moreover, the syndromal dimensions of hyperactivity and increased impulsivity are increasingly being recognized as highly persistent into adulthood and are associated with considerable risk for psychiatric comorbidity such as depression and substance use disorder as well as failure in psychosocial adaptation. 2 Although the substantial heritability of ADHD is documented in numerous family, twin and adoption studies, with estimates up to 80%, genome-wide linkage scans and candidate gene studies have failed to reliably identify ADHD-associated genes. 3 Hebebrand and co-workers 4 reported evidence for a risk haplotype at the dopamine transporter (DAT/SLC6A3) locus based on a linkage scan and subsequent finemapping of chromosome 5p13.…”

“…The ADHD-ODD group showed histrionic, narcissistic, aggressive-sadistic and negativistic traits, whereas the ADHD-ODD-co-morbid group had a combination of avoidant, narcissistic, antisocial, aggressive-sadistic, negativistic and self-defeating traits. Jacob et al (2007) applied the Revised NEO Personality Inventory (NEO-PI-R) in 372 adults with ADHD and 454 adults without ADHD. Patients with ADHD had higher neuroticism and lower extraversion, openness to experience and conscientiousness.…”
