Clozapine Versus “High-Dose” Olanzapine in Refractory Early-Onset Schizophrenia: An Open-Label Extension Study

Abstract: Objective: A recent 12-week controlled comparison demonstrated the superiority of clozapine to "high-dose" olanzapine in adolescents with treatment-refractory schizophrenia. In the present study, the authors conducted a 12-week, open-label, follow-up study to examine changes in lipid and glucose metabolism in youths maintained on clozapine and to determine whether patients who were previously randomized to high-dose olanzapine (up to 30 mg/day) responded to clozapine. Method: Thirty three (14 clozapine, 19 ola… Show more

“…The criteria align with recognized definitions for overweight and obesity in children (19). The excess weight gain noted with psychotropic medications is associated with an increased risk for hyperglycemia, insulin resistance, exacerbation or onset of diabetes, elevated triglycerides and cholesterol levels (7,8,(29)(30)(31)(32)(33). Other associated conditions of obesity include low selfesteem, which increases the likelihood of high-risk behaviors that can worsen underlying mental health conditions (11).…”

“…A troublesome side effect of many of these medications is weight gain (4,(6)(7)(8). Children can gain as much as 4-10 kg in a 10-week period (6).…”

“…In comparison, rates of neurological side effects with quetiapine are no different from placebo (DelBello et al 2002(DelBello et al , 2009Connor et al 2008). Neurological side effects from clozapine were not encountered in randomized trials comparing it with olanzapine (Shaw et al 2006;Kumra et al 2008) and haloperidol (Kumra et al 1996). Data on neurological complications with ziprasidone in children are scarce; one placebo-controlled trial described akathisia in 1 of 16 subjects randomized to ziprasidone therapy (Sallee et al 2000).…”

“…12 However, our use of olanzapine 280 mg/d exceeds by nearly 10 times the maximum of 30 mg/d employed in 1 recent study of ''high-dose'' olanzapine in schizophrenia. 13 Hospice and palliative care practitioners should be familiar with substantial dose escalation of medication dosage when justified by the presence of refractory symptoms. Certainly, opiates are administered at extraordinarily high doses near the end of life when pain is refractory to standard doses, and the primary goal of treatment is comfort.…”
