Closure of Oroantral Communication (OAC) with AlloDerm (Regenerative Tissue Matrix) – a case report

Abstract: Oroantral communication (OAC) is a routine complication in oral surgery. This can lead to nasal regurgitation, sinus infection if not treated quickly. Multiple techniques are used in managing this condition. Author describes a technique of closing OAC with AlloDerm (regenerative tissue matrix, Biohorizons, USA). The wound healed uneventfully. This technique appears to be a simple and time-saving technique and can be used by dentists with minimal surgical training.

“…Flap closures have been preferred to close an OAF; buccal advancement, palatal rotating flap, and buccal fat pad grafting are the most commonly used techniques 9 . Despite many advantages, these local flap closures have several drawbacks, such as postoperative pain, a prolonged healing period, a reduction in the depth of the buccal sulcus, the need for vestibuloplasty 10 , risk of flap failure, and an inability to repeat the same flap surgery 11 . To overcome these disadvantages, various alternative techniques have been reported, such as third molar transplantation 12 , the Bio-Oss-Bio-Gide sandwich technique 13 , hydroxyapatite blocks 14 , and AlloDerm closures 11 .…”

“…11 Although surgical techniques have many advantages, several drawbacks have been reported in previous studies: 1-Requirement of operator's skill and experience; 2-Loss of buccal vestibular depth; 3-Complication in prosthetic treatment plan; 4-Postoperative pain and discomfort of the patient; 5-Failure possibility of the flaps and 6-Inability to repeat the same flap technique in case of failure. 17,18 Due to the mentioned disadvantages of surgical procedures, various alternative techniques for OAC closure have developed recently. These contain use of alloplastic materials (bio-absorbable root analogues, hydroxyapatite), auto-transplantation (bone transplant, third molar transplant), allo-transplantation (fascia lata, dura mater, Human amniotic membrane), xenografts (bone materials, Collagen), prosthetic management and guided tissue regeneration techniques including autologous platelet or leukocytes (platelet-rich fibrin and platelet-and leukocyte-rich fibrin).…”
