Closure of Chronic Bronchopleural Fistula Using Atrial Septal Occluder Device

Abstract: Chronic persistent bronchopleural fistulae (BPF) are challenging management problems. The management of BPF includes various surgical and medical procedures and of great value is the use of bronchoscopy and different devices. In high-risk surgical patients, bronchoscopic procedures serve as a temporary bridge until the patient's clinical condition improves, whereas in other patients bronchoscopic procedures may be the only option. We present a case of postoperative chronic BPF closed by a less invasive interve… Show more

“…3,4 Previously published reports have also documented the use of Amplatzer devices in BPF closure after pneumonectomy. 5 The management of postpneumonectomy BPF is challenging. This case adds to the growing literature reporting on endoscopic, conservative management of this morbid complication and demonstrates the ease, feasibility, and success of conservative management of a BPF using an AV-PII under conscious sedation guided by flexible bronchoscopy and real-time fluoroscopy.…”

“…12 Some authors reported different closure methods such as atrial septal occluder device or Amplatzer device that could be used successfully to close BPF. 13,14 Bobocea et al 15 reported that videomediastinoscopic transcervical approach could be used successfully for BPF in selective cases. We think that the most accurate management for BPF is the individualization according to patients' general condition (sepsis, performance status, survey etc.)…”

“…Endobronchial occlusion methods are now performed routinely with different bronchoscopes and a long list of embolic agents such as blood clots, gelatin, silicon rubber plugs, lead fishing sinkers, various sponge materials, tetracyclines, fibrin glue, cyanoacrylate, detachable balloons, metallic coils, stents, and septal occluder device with successful outcomes.
