Clonal Relationship of Classical Hodgkin Lymphoma and Its Recurrences

Abstract: Purpose: We investigated whether the first and all subsequent manifestations of Hodgkin lymphoma (HL) in a patient are clonally related.Experimental Design: We identified a collective of 20 patients with sometimes multiple HL recurrences. Relapses were classified as early, that is, within twelve months (eight events in seven patients) or as late, that is, later than one year after the previous neoplasm (24 events in 17 patients). Hodgkin and Reed-Sternberg cells were microdissected after CD30 staining using la… Show more

“…Traditionally, even if a late relapse occurs, a relapsing lymphoid neoplasm may represent a recurrence of the original clone. However, this concept has recently been questioned [7,8]. This is in part due to the treatment progress over recent decades.…”

“…Comparison of the morphology and immunophenotype of the primary lymphoma and the relapse cannot always be used as a surrogate for clonal comparison, as clonally related relapses can show a different morphology or immunophenotype ( 51 , 52 ) and clonally unrelated relapses can look similar to the primary lymphoma. In cHL, unrelated relapse can be suspected by a change of histologic subtype or an altered EBV association, but this is not absolute ( 53 ). Therefore, clonal comparison is an advised in lymphoma relapse with a long interval.…”

“…Finally, clonality assessment is highly informative to establish the clonal relationship of cHL recurrences, which has been investigated in only one study with a significant cohort size [61]. For 20 cHL patients with cHL recurrences, laser microdissection (LMD) was performed followed by Sanger sequencing of isolated HRS cells to compare tumorspecific clonotypes in the primary and subsequent cHL presentation.…”
