Clip placement after stereotactic vacuum-assisted breast biopsy.

Abstract: A localizing clip can be placed in proximity to the stereotactic biopsy site through an 11-gauge probe. Clip placement can enable accurate localization for surgical excision.

“…Removal of selectedsmaller lesionsthat were marked with a metallic clip has been judged on a two-view upright mammogram obtainedimmediately after clip placement(Ta ble4). With the 14-gauge probe, thelesionwas removedin 59% of biopsiesby Burbankand Forcier [21] and in 60% of lesions by Liber man et al [22]. A higher removal rate was foundwith the 11-gaugeprobe:79% [21] and 71% [22}, respectively. We cannotevaluateour removal rate after clip placement becausewe did not start obtaining the recently advised [21,22] upright mammogram after clip placement until October 1997.After the immediate mam mogram after clip placement, our follow-up advice is the same as for lesions not marked with a clip.…”

“…Most cases of displacement are believed to be caused by the so-called "accordion effect". 2,[4][5][6][7][8][9] During the SVAB procedure and subsequent marker placement, the breast is in compression. After the procedure, when released from compression, the breast resumes its normal shape and the marker can displace along the axis of compression (the z-axis).…”

“…Performance of these markers is variable, with reported rates of displacement ,10 mm ranging from 54 to 99% in the literature. 1,[4][5][6][12][13][14] However, some of these higher published numbers are thought to be inflated, as displacement was measured with the breast still under compression on the stereotactic table, before the accordion effect is thought to take place. 6,12,14 In 2003, the first marker embedded within a biocompatible polymeric material was introduced to the market, the MammoMARK™ (Mammotome, Cincinnati, OH).…”

“…Stereotactic large-core breast biopsy, which is currently used to diagnose NPBL, provide fragmented tissue specimens and almost total disappearance of the tumor, when it is small. [54][55][56] The mammographic disappearance of the lesion poses several problems when the lesion is malignant. The surgical intervention will not always be optimally guided by a clip at the center of the biopsy bed.…”
