18 F-FDG PET has been widely used in the management of malignant tumors. In gastric cancer, the status of human epidermal growth factor receptor 2 (HER2) predicts the response to anti-HER2 antibody therapy, and testing of HER2 expression is now routine in the management of gastric cancer patients. However, to date, the relationship between 18 F-FDG uptake and HER2 expression has not, to our knowledge, been investigated. In this study, we aimed to investigate whether HER2 expression is associated with 18 F-FDG uptake and whether 18 F-FDG PET/CT can be used to predict the HER2 status of gastric cancer. Methods: A retrospective analysis was performed on 64 gastric cancer patients who had undergone 18 F-FDG PET/CT before surgical resection. Tumor SUV max was calculated from the level of 18 F-FDG uptake. Results: No significant correlation was found between SUV max and HER2 expression in gastric cancer. However, when signet-ring cell carcinoma was excluded, SUV max was significantly higher in the HER2-negative group than in the HER2-positive group (8.619 ± 5.878 vs. 3.789 ± 2.613, respectively; P 5 0.021). Multivariate analysis indicated that SUV max and tumor differentiation remained significantly associated with HER2 expression (P 5 0.048 and P 5 0.028, respectively). HER2 expression was predicted with an accuracy of 64.4% when an SUV max cutoff of 6.2 was used. Conclusion: 18 F-FDG uptake by gastric cancer is associated with HER2 expression. 18 F-FDG PET/CT may be useful for predicting the HER2 status of gastric cancer and for determining the therapeutic strategy.

Objective: Human epidermal growth factor-2 (HER-2) overexpression has prognostic value in breast cancer. However, the significance of HER-2 positivity in gastric cancer is controversial. In this study, we investigated the frequency of overexpression of HER-2 and its relationship with clinicopathological findings and impact on survival in gastric cancer. Material and Methods:Gastric cancer patients, operated in Marmara University Faculty of Medicine, Pendik Training and Research Hospital, General Surgery Department, between January 2012-December 2013 were enrolled in this study. Medical records were retrospectively evaluated. Tissue samples were stained by immunohistochemistry (IHC) method, and were followed by fluorescence in situ hybridization (FISH) in those with positive results. HER-2 expression rates and its association with other histopathological features and survival have been analyzed.Results: 135 patients were enrolled in the study, with 88 (65%) male and 47 (35%) female patients. The median age was 61 (29-84) years. Only 11 patients (8%) were positive for HER-2. HER-2 positive patients were similar to negative patients in terms of age, gender, tumor size, tumor location, tumor T stage, lymph node metastasis, histological type, differentiation, lymphovascular invasion, perinodal, perineural invasion and stage. No significant difference was detected on 1 and 2-year overall and disease-free survival rates between receptor positive and negative groups. Conclusion:Consistent with the literature data, HER-2 positivity rate in this study was approximately 8%, but this positivity has not been found to be associated with either clinical and pathological parameters or overall and disease-free survival.

Abstract. The presence of Her 2/neu has been reported in gastric cancer, but its impact on patient survival remains unclear. The purpose of this study was to investigate the expression of Her 2/neu in gastric cancer. a total of 218 paired resected gastric cancer and corresponding normal specimens were collected. Her 2/neu protein expression was assessed by immunohistochemical staining. The correlation between Her 2/neu expression and patient clinicopathological parameters was evaluated and the prognostic significance of Her 2/neu expression was assessed by univariate and multivariate analyses. Forty-one out of 218 (18.8%) gastric cancer specimens showed Her 2/neu-positive expression. no relationship was found between membranous Her 2/neu expression and clinicopathological parameters. However, Her 2/neu expression was correlated with poorer overall survival (p<0.001). in multivariate analysis, Her 2/neu expression was a significant independent prognostic predictor of gastric cancer (p<0.001), and was associated with poor survival in gastric cancer patients. These data indicate that Her 2/neu may play a major role in the therapeutic management of gastric cancer. Introductiongastric cancer is the second leading cause of cancer-related death worldwide (1). Surgery remains the mainstay of any curative treatment. unfortunately, in advanced gastric cancer, currently available treatments including surgery, chemotherapy and radiotherapy have limited success. Since surgery is an insufficient treatment for most patients, developing active drugs against advanced gastric cancer is required (2,3).It has been reported that trastuzumab has significant antitumor activity related to Her 2/neu expression in gastric cancer cells and xenograft models (4-6). This finding has prompted investigation of the potential clinical benefits of trastuzumab in gastric cancer. However, conflicting results have been obtained in studies on Her 2/neu and its relationship with prognosis in gastric cancer. Some studies have reported that Her 2/neu overexpression is a poor prognostic factor for gastric cancer (7-10), while others have failed to find any association between Her 2/neu overexpression and prognosis (11,12).
