Clinicopathologic features and treatment outcomes of patients with fibrillary glomerulonephritis

Abstract: Fibrillary glomerulonephritis (FGN) is a diverse glomerular disease with poor renal prognosis. The optimal therapeutic approach remains undetermined, as treatment outcomes vary across different studies. We retrospectively reviewed the medical data of 10 patients diagnosed with biopsy-proven FGN at our center between 2004 and 2019. Clinical and histological features, as well as therapeutic regimens and treatment response, are reported. The patients were predominantly men (2.5/1 men-female … Show more

“…Patients with FGN classically present in middle age (average 49-61 years) with decreased kidney function (average serum creatinine 2.1-3.7 mg/dL), microscopic hematuria (in 39-95%), and proteinuria (average 4.1-7.3 g/day) and occasionally frank nephrotic syndrome (in 15-52%) [3-8, 11, 12, 15, 16]. Approximately 41-74% of FGN patients are female, with a female predominance in US-based cohorts (female-to-male ratio 1.2-2.8:1) [12] and a male predominance reported in studies from France, Switzerland, and Greece [15][16][17]. FGN is rarely found in young adults [18,19].…”

“…These effects lead to loss of DNA expression, cell cycle arrest, inhibition of replication, and, ultimately, death often via apoptosis. The rationale for use of these agents in the treatment of GD is inhibition of immune cell development and function resulting in strong immunosuppression [ 44 ]. Two commonly used alkylating agents in GD are cyclophosphamide (CYP) and chlorambucil.…”

“…Most cases occur in Caucasian patients in the sixth decade of their life, with gender predominance in females (male-to-female ratio 1:1.8). However, specific reports reveal male predominance in some countries such as Greece [3][4][5].…”
