This study aimed to describe and compare breastfeeding progression, infants' feeding behaviours, maternal feeding difficulties, and mothers' usage of breastfeeding interventions for singleton late preterm (LPT) and term infants. A further aim was to identify associated factors for exclusive breastfeeding at breast at 1 month in LPT infants.This was a cohort study where mothers of LPT infants from a neonatal unit (n = 60), LPT infants from a maternity unit (n = 62), and term infants from a maternity unit (n = 269) answered a questionnaire approximately 1 month after delivery. Findings showed no significant differences in exclusive breastfeeding at breasts between LPT infants admitted to the neonatal unit compared with the maternity unit, during the first week at home (38% vs. 48%), or at 1 month of age (52% vs. 50%). Term infants were more likely to be exclusively breastfed at the breast (86% and 74%, p < 0.05) compared with LPT infants. Multiple regression analysis showed that usage of a nipple shield, not feeding breast milk exclusively during the first week at home, or feeding less than 10 times per day at 1 month were statistically significant for not exclusively breastfeed at the breast. A protective factor was the mothers' experience of having an abundance of milk during the first week at home. In conclusion, LPT infants are less likely to be exclusively breastfed at the breast than term infants, highlighting the need for further research to guide interventions aimed at optimising exclusive breastfeeding rates.

ÖZET Preterm bebeklerde emme, yutma, nefes alma döngüsünün birbiriyle eş zamanlı çalışamaması ve bu çoklu sistemlerin kontrol edilememesi, yutma ve solunum işlevlerinin ortak alanda bulunması gibi nedenlerle beslenme güçlüğü, yetersiz kalori alımı, gelişim geriliği, taburculuk süresinin uzaması ve bakım yükü ile birlikte maliyetin artması olarak karşımıza çıkmaktadır. Erken doğan bebeklerde yeterli besin ve kalori açığının kapanması, hem büyüme geriliğini ve pretermliğe bağlı ölümleri önemli ölçüde azaltmakta hem de yatış süresini kısaltmaktadır. Geçmişte yenidoğanların beslenmesi, stres kaynakları göz önünde bulundurulmadan sadece alınan besin miktarı ile değerlendirilirken; günümüzde, bebeğin beslenme sırasında verdiği ipuçlarını takip ederek davranışsal tepkilerini yorumlama ve bebeğin beslenme becerilerini geliştirmeye odaklı ipucu temelli beslenme yaklaşımı benimsenmektedir. Bu çalışmada, preterm bebeklerin kanıta dayalı oral beslenme yöntemlerden birisi olan ipucu temelli beslenme modeli sunulmuştur. Anah tar Ke li me ler: Besleme; ipuçları; prematüre; yenidoğan hemşireliği ABS TRACT The problems such as feeding difficulties, insufficient caloric intake, developmental delay, prolonged discharge time, the care burden and increased costs in preterm infants arise because of the non-simultaneous sucking, swallowing and breathing cycle, inability to control these multiple systems and the presence of swallowing and respiratory function in the common area. To ensure adequate nutrient and calorie deficit in premature babies reduce both growth retardation and mortality caused from prematurity and also shorten the time of hospitalization. In the past, the feeding of newborns was evaluated only by the amount of nutrients taken, without considering the sources of stress; today, however, a cue-based feeding approach is adopted, focusing on improving the feeding skills of the baby by interpreting the behavioral responses of the baby and following the cues given by the baby during the feeding. In this review article presents a cue-based feeding model, which is one of the evidence-based oral feeding methods in preterm infants.

Background and AimsOral feeding for preterm infants has been a challenging issue globally. In an effort to enhance the effectiveness of oral feeding in preterm infants, oral motor intervention (OMI) was developed. Present systematic review and meta‐analysis study aims to examine the impact of various OMI techniques on key outcomes, including body weight at the time of discharge, the duration required to achieve independent oral feeding, and the length of hospital stay for preterm infants.MethodsA systematic search of the literature was performed across various databases such as PubMed, Scopus, and Web of Science and Google Scholar up to September 28, 2023. Quality assessment was conducted using the Joanna Briggs Institute (JBI) checklist. The overall effect measure was calculated using a random‐effects model and was presented as the standard difference of the mean (SDM), accompanied by the standard error and a 95% confidence interval (CI). We used I2 statistic for investigating the heterogeneity between studies. Data analysis was performed by CMA software (Version 2).ResultsFinally, 22 articles included in this review. The overall effect for body weight at discharge was found to be statistically significant in the prefeeding oral stimulation (PFOS) (SDM = 7.91, 95% CI: 5.62, 10.2, p = 0.000, I2 = 86.31) and Premature Infant OMI (PIOMI) (SDM = 3.71, 95% CI: 0.72, 6.69, p = 0.01, I2 = 96.64) groups versus control group. The overall effect of independent oral feeding was significant for PFOS‐only (SDM = −0.64, 95% CI: −1.1, −0.17, p = 0.007, I2 = 75.45), PIOMI only (SDM = −1.48, 95% CI: −2.49, −0.46, p = 0.004, I2 = 93.73) and nonnutritive sucking (NNS) only (SDM = −0.53, 95% CI: −0.76, −0.30, p = 0.001, I2 = 0) groups versus control groups. The overall effect of length of hospital stay was significant for NNS group (SDM = −0.45, 95% CI: −0.67, −0.23, p = 0.067, I2 = 0) and PIOMI group (SDM = −0.42, 95% CI: −0.69, −0.15, p = 0.002, I2 = 20.18) versus control group.ConclusionAmong OMIs, the PIOMI approach generally exhibited a more favorable impact on body weight gain at discharge, the duration required to achieve independent oral feeding, and the length of hospital stay.
