allergy centres in India prepare their own acetone extracts, which is cumbersome, and involves manual plucking of leaves and trained personnel. This exposes them to risk of contact sensitization. The extracts degrade and lose their potency over time which can cause false negative reactions. In our experience, the plant extracts need to be replaced every 3 months. In contrast, the better preserved ISS allergens retain their potency till their expiry date when stored properly. In this study, parthenium 15% in ISS showed a positivity rate similar to extract. However, discordant reactions were noted between them, with 15% (9/60) cases were positive only to extract 2% and not to parthenium 15%. Therefore, acetone extracts should remain in use. Positive reactions to parthenium 15% but negative to extract 2% can be due to the higher concentration of the former. Despite a higher concentration, there were no irritant reactions to parthenium 15% in the PCD controls, signifying its low irritant potential. However, as no late readings were done, the possibility of parthenium 15% having a higher risk of active sensitization cannot be ruled out. Larger studies are, therefore, needed to confirm our results.In conclusion, there is no significant difference in the positivity rates between parthenium 15% and extract 2%. However, their use together yields better pick-up rates, as it can detect cases missed by either one.

Correspondence e273recent meta-analysis. 4 Our patient, however, did not have Koebner's phenomenon and confetti lesions. Recent data suggest oral corticosteroids, that is, 4 mg dexamethasone twice weekly, narrow-band-UV phototherapy (three times per week), and topical clobetasol-propionate (five times per week), as the most efficient treatment option. 5 Our patient refused oral corticosteroids. As she had a low Fitzpatrick skin phototype, we decided not to perform phototherapy but to treat her with topical corticosteroids followed by topical calcineurin inhibitors.
