“…Various equations exist for calculating GFR producing different results . The Cockcroft‐Gault equation is most common and has been in routine use in clinical practice since 1973 . Although results by means of the Cockcroft‐Gault equation tend to be higher than true GFR and estimates by other equations, studies indicate that it actually underestimates GFR in the elderly.…”
Lack of dose adjustment for renal impairment seems to be a common problem, even in outpatients. However, the differences in methodologies used in these studies hampered any direct comparison. Accepted and comparable standards regarding the drugs included in the studies as well as estimation of renal function would be beneficial.

“…Introduction: Although in elderly people renal function may be impaired, little is known about the prevalence of renal impairment in nursing home residents. Percentages between 44 and 91 of renal dysfunction have been reported (Papaioannou, et al 2001;Nygaard, et al 2004), while on a geriatric ward in a hospital in 80% of the patients the creatinine clearance was found to be less than 60 ml/min (Martin, et al 2005). As information on renal dysfunction seems relevant for dose adjustments of prescribed medication, a survey on renal function was performed in three representative Dutch nursing homes.…”
“…In een Scandinavisch onderzoek wordt bij 91% van de ouderen een matige tot ernstige daling van de Glomerulaire Filtratie Rate (GFR 15-59 ml/min) gevonden 1 . De farmacokinetiek van medicamenten die voor hun uitscheiding afhankelijk zijn van de nier verandert, zodat aanpassing van medicatie nodig kan zijn.…”
