Clinical validation of controlled exposure to birch pollen in the Environmental Exposure Unit (EEU)

Abstract: BackgroundThe Environmental Exposure Unit (EEU) in Kingston, Ontario, Canada is a controlled allergen challenge facility (CACF) that has been previously clinically validated for the use of ragweed and grass pollen in clinical studies. In this study we aim to validate the use of birch pollen to challenge allergic participants.MethodsA total of 59 volunteers were screened and 38 birch allergic participants and ten non-allergics completed the study, outside of tree pollen season. Participants had to have a minimu… Show more

“…38 The 4-item total ocular symptom score (TOSS) (each measured on a scale of 0-3; 3 being severe; total score range 0-12) is a composite symptom assessment of ocular symptoms (itching/burning, redness, watering, tearing, and puffiness and swelling). 39 The PNIF (L/min) is a measure of nasal patency or nasal congestion. At the screening NAC visit, TNSS, TOSS, and PNIF were measured pre-NAC and during NAC; for details, see the Methods section in this article's Online Repository.…”

“…Another limitation was combining allergic and healthy participants from several different EEU studies, which used either seasonal or perennial allergens. However, for each study, allergen concentrations and exposure times were previously determined and proven to generate a mean peak of six for total nasal symptom scores (TNSS; a composite score of sneezing, rhinorrhea, and nasal congestion and itching) (23)(24)(25). Participants achieving a TNSS of 6 is a standard for controlled allergen challenge facility studies (26).…”

“…As demanded by a recently published position paper from the European Academy of Allergy and Clinical Immunology (EAACI) it is important to compare the obtained results between the existing exposure chambers [ 37 ]. To the authors knowledge only two studies conducted in an Environmental Exposure Unit (EEU) in Kingston, Ontario have used PNIF as an outcome parameter in clinical trials [ 38 , 39 ]. Both studies were clinical evaluations of the EEU for birch pollen and grass pollen exposure, respectively.…”
