In this paper a report is made on 23 patients with falciparum malaria who were successfully treated with sulfamonomethoxine. Evidence has been presented: (1) that its effect on falciparum malaria is very remarkable, (2) that the intravenous administration is more effective than the oral one, (3) that also clinically chloroquine insensitive malaria responds to it, and (4) that no "relapse" is detected.

O autor apresenta uma revisão sôbre o mecanismo de ação dos sulfamídicos ãe ação prolongada, tecendo considerações sôbre a sua distribuição no organismo n sua atividade antibacteriana. Aprecia principalmente as características farmacodinâmicas da sulfametoxipirazina considerando-a superior aos demais sulfamíâicos ensaiados, devido à sua lenta eliminação e à sua reduzida ligação às proteínas plasmáticas, características que, aliadas à sua pequena toxicidade e atividade bacteriana elevada, perm item o seu emprego terapêutico em doses baixas.No campo | da farm acoterapia clínica e em p articu laí no da quim ioterapia dos pro cessos infectjiosos revestem-se de grande im portância as aquisições feitas nestes úl timos anos no domínio da farm acocinética. P a ra correta im postação terapêutica é necessário, de fato, determ inar p ara cada quimioterápico:1 -dose ótim a que deve ser adm inistrada p ara obter ao nível do foco inflam atório concentração eficiente de m edicamento ativo; 2 -dose de m anutenção que deve ser adm inistrada a intervalos de tempo pré-fixados p ara m an ter invariada esta con centração .A determ inação dêstes parâm etros é de particular interêsse no tratam en to com de rivados sulfamídicos (SA) porque a ação quim ioterápica dêstes compostos e dos me dicam entos bacteriostáticos em geral é uma função da interação de dois fatores princi pais: concentração e tem po.E sta relação é válida dentro dos limites de um determ inado âm bito de valores por que é impossível reduzir um ou outro fator abaixo dos valores do lim iar sem compro m eter a ação terapêutica.A atividade quim iotôrápica de um SA depende, portanto, de sua atividade antibactérica intrínseca e de suas característi cas farm acodinâm icas.A atividade antibactérica intrínseca é condicionada, conforme h á tempos se sabe (26), da analogia estrutural dos SA com o ácido-p-amino-benzóico (PAB); e dificil m ente pode alcançar em novos compostos valores superiores aos da sulfadiazina, cuja constante de dissociação ácida é muito vi zinha à do PAB (3).As características farm acodinâm icas co mo a atividade terapêutica sôbre as infec ções experim entais variam com o variar dos anéis heterocíclicos e da posição dos substituintes a êles ligados, como é evidente nas p. am inobenzen-sulfamidas de am inas heterocíclicas (15).Os trabalhos de Dost (11) e de Krüger-Thiem er e cols. (16,18,19) perm itiram re sum ir em um a expressão m atem ática única os diversos grupos de fatores e calcular as doses de m edicamento necessárias e sufi cientes p ara obter terapêutico ótimo. “…Pyrimethamine alone is effective against several but not all chloroquine-resistant falciparum strains (10,24). Two long-acting sulfonamides, sulfadoxine (Fanasil) and sulfalene (Kelfizina), as well as two short-acting ones, sulfadiazine and sulfisoxazole, recently have been used alone or in combination with pyrimethamine in the treatment of chloroquine-resistant falciparum malaria with varying degrees of success (2,9,13). In 1969, Berman (5) reported that a combination of chloroquine, pyrimethamine, and sulfisoxazole produced radical cures in 76 patients at the Naval Support Activity Hospital, Da Nang, Republic of Vietnam.…”

During the course of antimalarial screening, it was discovered that sulfamethoxydiazine, a long-acting sulfanilamide extensively used in genitourinary tract infections, not only was effective against Plosmodium berghei infections in mice when administered alone but also was active when used in combination with chloroquine, in effect making it possible to use half as much of the latter drug as normally required to achieve the same results. The triple combination of chloroquine, sulfamethoxydiazine, and pyrimethamine, when administered in a ratio of 30:10:1, was found to be potentiating against both blood-induced and sporozoite-induced P. berghei NK6e infections. Mean effective dose values were calculated for chloroquine, sulfamethoxydiazine, and pyrimethamine against blood-induced P. berghei infection, and when a combination of the three drugs was administered therapeutically in the ratio given above, only one-tenth as much chloroquine, one-thirtieth as much sulfamethoxydiazine, and one-sixtieth as much pyrimethamine were needed to cure 50% of murine infections as was needed for each drug alone. The triple combination also showed enhanced activity against lethal sporozoite-induced P. berghei NK65 infection in A/J strain mice.
