Over a 2 year period from 2013 to 2014, we performed a clinical study at our center of 1,068 patients with a chief complaint of dizziness/equilibrium disorder. The ratio of males to females was 2:3, with the highest proportion of men in their 60 s. The distribution of females was bimodal, with the highest proportions of females in their 40 s, 60 s, and 70 s. The majority of referrals were directly received at our department, but the next most was introductions from otolaryngology practitioners. Peripheral vestibular disorder accounted for approximately 77 of all cases, among which BPPV, Ménière's disease, and vestibular neuritis were observed in ascending order. Cerebrovascular accidents were observed in approximately 9 of all cases. We demonstrate the importance of the use of diagnostic criteria during examinations for dizziness. A higher proportion of referred cases had BPPV and Ménière's disease and many referrals were from otolaryngology practitioners, which suggest that refractory cases are also included in the preset study population. The utility of nystagmus examination in the emergency room and during consultations may improve diagnostic accuracy in such cases. Although undiagnosed cases are included in any clinical study, we believe further improvements in diagnostic techniques will have greater utility than the use of follow-up for diagnostic confirmation.

Vertigo or dizziness is infrequent in pediatric patients. In this study, we retrospectively investigated the prevalence and etiology of vertigo in children. The subjects were 127 patients who were less than 16 years old (at the first visit) and visited the university hospital of Kyoto Prefectural University of Medicine between January 2008 and March 2022, with the chief complaint of vertigo or dizziness. The patients consisted of 53 boys and 74 girls, with a mean age of 11.2 years. The predominant diagnoses were orthopedic dysfunction (OD; 34 patients, 26.7%); psychogenic vertigo (16 patients, 12.6%); vestibular migraine of childhood (VMC; 16 patients, 12.6%); recurrent vertigo of childhood (8 patients, 6.3%); Meniere's disease (5 patients, 3.9%); delayed endolymphatic hydrops (5 patients, 3.9%), hypotension due to underlying conditions (5 patients, 3.9%), and others.The percentage of patients with OD was the highest at 39.7% in junior and senior high school students. The percentage of patients with psychogenic vertigo was the highest at 16.2% in late elementary grades. The percentage of patients with VMC was almost the same in students from elementary school to high school. The percentages of patients with inner ear anomalies and hypotension due to underlying conditions were high in early elementary students.Triggers were observed in 63 patients: psychogenic stress, sleep deprivation, chronic fatigue, excessive gaming, menstruation, pain due to other diseases, water deficiency, and adverse weather. The most frequent cause was bullying in the patients with psychogenic stress. Extracurricular activities were the main cause in both patients with sleep deprivation and those with chronic fatigue.
Vertigo and dizziness are common symptoms encountered in clinical settings. We had examined 40 cases with a chief complaint of vertigo and dizziness that were effectively treated with Kampo medicines for 5 years. Here, we present 4 typical cases and report on the outcomes of these cases. A majority of the patients reported here had dizziness as the main complaint and were female. About 35% of the cases improved in no more than 7 days, and 50% of the cases were cured within 14 days. Ryokeijutsukanto (45%) and shinbuto (55%) are the most commonly prescribed Kampo medicines. We had to treat the patients with not only a single, but also multiple Kampo medicines because of tangled yin and yang.
