Clinical Significance of Immunoglobulin Deposition in Leukocytoclastic Vasculitis

Abstract: In patients with cutaneous LCV, IgA is the most common immunoglobulin found by DIF. IgA deposition, but not IgM or IgG, is predictive of associated renal and gastrointestinal organ involvement by vasculitis. No association between the type of immunoglobulin and preexisting infection or malignancy was found. DIF results add information that is clinically relevant to the diagnosis and management of LCV.

“…The five patients with non‐IgA vasculitis that had IgA positivity on DIF also had positive IgM and fibrinogen as the predominant immunoconjugates. IgA deposition in LCV has been found to be correlated with the extent of organ involvement and should prompt closer follow‐up by clinicians …”

“…In consideration of the probable clinical overlap between hypersensitivity vasculitis and IgAV, differentiating between these two entities is imperative when assessing and predicting renal or GI involvement . Consequently, the clinical significance of the presence or absence of IgA deposition found by direct immunofluorescence of the cutaneous biopsy specimen should not be overlooked, as it enhances the diagnostic specificity …”
