Clinical Significance of Atypical Glandular Cells in the Bethesda System 2001: A Comparison with the Histopathological Diagnosis of Surgically Resected Specimens

Abstract: Forty-one patients diagnosed with atypical glangular cells (AGC) underwent surgery, and the histopathological diagnosis results for the resected specimens and the clinical features were analyzed. Out of 41 patients, final pathological diagnosis was endometrial cancer in 13 patients, cervical adenocarcinoma in 8, AIS in 7, CIN3 in 6, others in 2, and no lesions in 5. In comparison with previous reports, malignant or premalignant lesions were detected more frequently in patients with AGC who underwent surgery. W… Show more

“…As in the literature [5,6,10,11], the majority of our cases were histologically benign, and there was no significant difference between the two periods. We seem to see more endometrial cells, but the majority are benign on follow-up, and the differences are not significant.…”

“…Other studies have found that subcategorization of AGUS may give a higher yield of significant lesions in specific subgroups [11,[13][14][15] We can assume that the risk of an underlying malignant or premalignant lesion is of the same magnitude in Norway, and our current practice of follow up is justified and in compliance with international studies [10,13,15].…”

“…The development of CC occurs over a number of years and its complexity presents clinical challenges in patients screening and treatment. Currently, The Bethesda System (35), which is a tool that is used to report Pap smear results for cervical cytologic diagnoses, provides useful data that allows research into the epidemiology, biology and pathology of cervical lesions; however, its diagnostic value remains poor (36). Instead, direct biopsy remains the gold standard for diagnosis.…”

“…Verdoodt et al [17] conducted a meta-analysis to examine the accuracy of hrHPV testing for identifying or excluding (pre)cancerous lesions of the uterine cervix and reported that 19.8% of women with AGC were classified as CIN2+/AIS+. The prevalence of CIN2+/AIS+ was 22-48% in previous studies examining 50-100 cases of AGC, whereas 1 Japanese study reported that 87.5% of women diagnosed with AGC were classified as CIN2+/AIS+ [18]. In our study, 58.5% (24/41) of women with AGC were classified as CIN2+/ AIS+ and 91.7% (22/24) were CIN2+.…”
