Clinical Significance of Anti-HEV IgA in Diagnosis of Acute Genotype 4 Hepatitis E Virus Infection Negative for Anti-HEV IgM

Abstract: Anti-HEV IgM is a diagnostic for recent or ongoing HEV infection. However, some patients with acute hepatitis E (AHE) negative for anti-HEV IgM in acute period were often observed in clinical practice. In this study, we constructed the anti-HEV IgA indirect ELISA assay to evaluate the significance of anti-HEV IgA. The specificity of anti-HEV IgA was 99.6%. Among 245 AHE patients, 84 samples from 84 patients were positive for HEV RNA. The positive rate of anti-HEV IgA, anti-HEV IgM and anti-HEV IgG in 84 sample… Show more

“…Marked variability in the sensitivity and specificity of HEV assays complicates the interpretation and comparison of serologic studies (193)(194)(195)(196)(197)(198)(199), and this fact should be borne in mind when considering the serologic data presented in this section and elsewhere. Studies with early anti-HEV IgM and IgG commercial assays often failed to detect seropositive individuals, undercounting the seroprevalence (200).…”

“…Case reports and clinical studies from Europe have also raised suspicions about serologic cross-reactivity in the presence of other viral infections, such as Epstein-Barr virus (EBV) or cytomegalovirus (CMV) (201,202). Several recent serologic assays and other diagnostic methods have been found to have improved performance in a wide range of settings (194,198,(203)(204)(205)(206)(207).…”

“…Currently, a rapid immunochromatographic test is available to detect IgM antibodies in acute hepatitis E that works on the basis of reverse-flow technology and yields results in a matter of 2–3 min [63]. Anti-HEV IgA antibody detection could be used as a supplementary means to prove the acute disease if an anti-HEV IgM antibody test fails to do so [64]. It has not been determined whether anti-HEV IgA plays a major role in the detection of HEV nosocomial outbreaks or not.…”

“…Previous studies have reported that the detection of anti-HEV IgA is a convenient complementary marker for the diagnosis of HEV infection [88,89,90], especially regarding the enhanced specificity of a combination of both anti-HEV IgM and IgA immunoglobulins. Like IgM, IgA anti-HEV antibodies appear during acute hepatitis E. However, Herremans and coworkers [90] reported infections with HEV genotype 3 without an increase of IgA antibodies.…”
