Clinical significance of "anti-HBc alone" in human immunodeficiency virus-positive patients

Abstract: AIM:To determine the prevalence and clinical relevance of isolated antibodies to hepatitis B core antigen as the only marker of infection ("anti-HBc alone") among human immunodeficiency virus (HIV) type-1 infected patients. Occult hepatitis B infection frequency was also evaluated. METHODS:Three hundred and forty eight histories from 2388 HIV-positive patients were randomly reviewed. Patients with serological markers of hepatitis B virus (HBV) infection were classified into three groups: past hepatitis, "anti-… Show more

“…In some individuals, the only serological marker of HBV infection is the presence of anti-HBc antibodies (10,24), and thus detection of "anti-HBcAg alone" could reflect unrecognized "occult" HBV infection and physicians should consider investigating such patients with HBV molecular tests (21). Additionally, isolated anti-HBc can be used as a marker to assess the risk of HBV reactivation in patients undergoing therapy that could result in immunosuppression or patients who are HIV positive (17) or hepatitis C virus (HCV) positive (25).…”

“…Careful follow-up is required in order to start, complete and booster vaccination schedules in HIV patients, not only for HBV but probably also for other vaccine antigens. For all these reasons, the doctor who is informing you, an expert in the management of pediatric HIV infection, considers it appropriate for you to take part in the Implementation of Occult Hepatitis Screening in the 152 Spanish Cohort of HIV-Infected Pediatric Patients study that is going to be carried out in several Spanish pediatric hospitals.…”

“…However, the results are not conclusive since liver biopsy for HBV DNA intrahepatic detection was not performed, and children participating in the study showed a high median age and good clinical and immunological status, as good as those in similar studies. Thus, the interpretation of the HBV marker profile in the 6 patients with probable OBI in the present study remains controversial in terms of its being a surrogate marker for HBV/HIV coinfection, or possibly not in the event of additional immunosuppresion (144,152,157,160). …”
