Clinical reasoning processes: unravelling complexity through graphical representation

Abstract: The MOT model of clinical reasoning processes has potentially important applications for use within undergraduate and graduate medical curricula to inform teaching, learning and assessment. Specifically, it could be used to support curricular development because it can help to identify opportune moments for learning specific elements of clinical reasoning. It could also be used to precisely identify and remediate reasoning errors in students, residents and practising doctors with persistent difficulties in cli… Show more

“…[15] What experts seek to uncover is whether the current situation bears the same characteristics than that of its usual representation, as mapped by their scripts. [9,16] It is important to clarify that experts also develop back-up potential hypotheses and that the recursive exercise of clinical reasoning leads to the confrontation and hierarchizing of these hypotheses. [16,17] Their reasoning also relies on a situational awareness of the key elements of a health problem.…”

“…[9,16] It is important to clarify that experts also develop back-up potential hypotheses and that the recursive exercise of clinical reasoning leads to the confrontation and hierarchizing of these hypotheses. [16,17] Their reasoning also relies on a situational awareness of the key elements of a health problem. Thus, clinical reasoning relies on a wealth of professional knowledge developed over the years, which is quickly retrievable, as well as on more deliberate reflections on professional practice.…”

“…To assess the doctor's reasoning, there are techniques such as "concept mapping" 8 and the "think aloud" 9 protocol, the latter being the most widely used. In it, the verbal data obtained can be used to study the cognitive process under specific and controlled situations 9 .…”

“…1 Physical therapists use LBP classification systems, which include, but are not limited to; evaluation of baseline symptom behavior, examination of movements, posture, neurological and neuro-dynamic testing and assessing spinal stability. 2 Several detailed classifications have been developed, with the belief that subgrouping people with LBP is important for both clinical and research purposes. 3,4 They include, but are not exclusive to, systems such as McKenzie's mechanical diagnostic therapy (MDT).…”
