Objectives: Accidental poisoning is one of the major medical emergencies in paediatric population, specially among under five children. However, there is still lack of clinicodemographic data mostly due to under-reporting of events. In this study, we analyzed the clinicoepidemiological profile as well as awareness of parents in respect to childhood poisoning cases admitted in our institute. Methods: Children aged up to 12 years admitted in our hospital with accidental poisoning between January 2020 to June 2021 were included in this study. We collected their demographic, epidemiological and clinical data, analyzed, and compared these with data from other parts of India as well as world. Results: About 87% of children were under 5 years of age with a male dominance (male: female was 1.3:1). Majority of children (65%) were from lower socioeconomic class and 66% of parents had no knowledge or ignorant about the poisonous substance. Hydrocarbons like-kerosene oil (44.5%) followed by drugs were the most common substance related to poisoning. Cough (23.7%) followed by vomiting (21%) was the most common presenting symptom and cardiorespiratory abnormalities were most common (49%) finding on clinical examination. About 5% of children died due to poisoning or its related effects. Conclusion: It can be said that negligence and ignorance from the side of caregivers play the most important role in such calamities. Community-based childhood poisoning prevention program and simple preventive measures like education to the parents, proper storage of household items, and proper placement of drugs could be helpful in preventing mortality and morbidity in children with poisoning.

Background: Accidental poisoning, though preventable, has continued to be one of the most prevalent medical emergencies among children in developing countries.Objective: To describe the clinico-demographic profile and outcome of children with hydrocarbon poisoning at a tertiary care teaching hospital from Eastern India.Methods: A retrospective analysis of the hospital case records of all children <15 years of age admitted with hydrocarbon poisoning from January 2015 to October 2018 was performed.Results: Out of 2658 total admissions, 21 children were admitted with hydrocarbon poisoning during the study period. The median age was 2.6 years (range, 1 to 14 years). Majorities (76%) were <5 years of age. The male/female ratio was 5:2. The majority belonged to rural areas (80.9%) and low socioeconomic status (71.4%). In all the cases, poisoning was accidental in nature. Kerosene (71.4%) and turpentine oil (24%) were the most common agents implicated. Majorities (95.3%) were symptomatic requiring hospitalization. Fever and vomiting were the most commonly observed symptoms (57%). Neutrophilic leucocytosis was observed in 62% of cases. Abnormal chest radiography was observed in 67% of cases. There was no mortality.Conclusions: The majority of the children with hydrocarbon poisoning are under five years of age with the accidental mode of poisoning in the current study. Kerosene was the most common agent. The outcome was excellent without any mortality.

ObjectiveTo determine the prevalence of pesticide, corrosive, drugs, venom and miscellaneous poisoning in India.SettingSystematic literature search was done in PubMed Central, Cochrane and Google Scholar databases for studies that satisfied the inclusion criteria. Systematic review and meta-analyses of all observational studies published in the English language from January 2010 to May 2020 were included in this review.ParticipantsPatients exposed to poisoning reported to hospitals were included.Primary and secondary outcome measuresThe prevalence of pesticide poisoning was analysed. The prevalence of poisoning due to corrosives, venom, drugs and miscellaneous agents, along with subgroup analysis based on age and region, was also determined. The percentage of persons with poisoning along with 95% CI was analysed.ResultsPooled analysis of studies revealed that pesticides were the main cause of poisoning in adults, with an incidence of 63% (95% CI 63% to 64%), while miscellaneous agents were the main cause of poisoning in children, with an incidence of 45.0% (95% CI 43.1% to 46.9%), among those presenting to hospitals. Pesticide poisoning was the most prevalent in North India (79.1%, 95% CI 78.4% to 79.9%), followed by South (65.9%, 95% CI 65.3% to 66.6%), Central (59.2%, 95% CI 57.9% to 60.4%), West (53.1%, 95% CI 51.9% to 54.2%), North East (46.9%, 95% CI 41.5% to 52.4%) and East (38.5%, 95% CI 37.3% to 39.7%). The second most common cause of poisoning was miscellaneous agents (18%, 95% CI 18% to 19%), followed by drugs (10%, 95% CI 10% to 10%), venoms (6%, 95% CI 6% to 6%) and corrosives (2%, 95% CI 1% to 2%).ConclusionsPesticide poisoning is the most common type of poisoning in adults, while miscellaneous agents remain the main cause of poisoning in children.PROSPERO registration numberCRD42020199427.
