Right hepatic artery (RHA) is a branch of the common hepatic artery; however, there are cases documented in the literature showing anatomical variations. Accessory RHA is an incidental finding during hepatobiliary and pancreatic surgery. This artery should be identified, carefully separated, and preserved during these surgeries. We encounter the variation while doing Whipple’s procedure in a 61-year-old patient diagnosed with carcinoma of the head of the pancreas. Intra-operatively, accessory RHA was present which was arising from the superior mesenteric artery. It was identified, carefully separated, and preserved. Variations in the origin of the artery may make it vulnerable to injuries during surgical procedures if due care is not taken.

Introduction: Hydatid disease is caused by a parasite infection induced by an echinococcus tapeworm. The hydatid cyst is one recognized cause of liver mass. It is a significant pathogenic, zoonotic, and parasitic illness (acquired from animals) of humans after consumption of tapeworm eggs produced in the faeces of infected dogs. Hydatid disease is a serious endemic health concern in various regions of the world. Cystic hydatid disease is most often associated with the liver (50–70%), although it can also affect the lung, spleen, kidney, bones, and brain. A hydropneumothorax occurs gradually. Case Presentation: A 35-year-old Man was taken to the Acharya Vinoba Bhave Rural Hospital with the chief complaint of abdominal discomfort, breathlessness (dyspnea) right side pain for 2 days. Approximately to the peritoneal cavity of the right chest by a hydatid cyst in the subscapular posterior part of the right lobe of the liver. For minimal ascites, a contrast study was conducted, revealing a multi-located hypodense cystic lesion spread across the abdomen was recognized as sign of hydatid disease. Conclusion: In a patient with a hydatid disease, physical and psychological therapy should be put together. In this study, we primarily focus on professional management and outstanding nursing care, which may give the comprehensive care that hydatid cyst requires. The complete health care team works together to assist the patient to restore his or her prior level of independence and happiness after a full recovery.

Introduction: The outbreak of coronavirus disease 2019 (COVID-19) has recently become a major problem affecting thousands of individuals around the world. It is understood that a significant proportion of patients infected with COVID-19 have disturbed liver function tests. This is a case report of a patient with liver cirrhosis and COVID-19. Presenting Complaints and Investigations: A 39 year old female patient was admitted in AVBRH on 06/02/2021 with chief complaint of distension of abdomen, breathlessness, shortness of breath, reduced urine output over last 3 months. She had skin lesions over the hand, foot, abdomen and back with itching since 4 months. Physical examination, blood investigations and abdominal ultrasound showed cirrhosis of liver with gross ascites seen clinically. She had mild splenomegaly and gall bladder was enlarged. Laboratory tests showed elevated total bilirubin level. In peripheral blood examination, RBC count was low (3.66m/cu mm), Haemoglobin level was 8.2 gm/dl, Platelets count was low (1.19 lakh per cu. mm). WBC count was 3600 cu mm. Doctor diagnosed this as the case of cirrhosis of liver with pemphigus vulgaris with COVID-19. Past History: 6 months ago, patient was admitted in Aarogyadham hospital Yawatmal with chief complaint of abdominal pain, loss of appetites, fever. On ultrasonography, she had splenomegaly for which she took the treatment. The Main Diagnosis, Therapeutic Intervention and Outcomes: This case was diagnosed as a case of COVID-19 with Cirrhosis of liver. Interferon alpha 2b solution was given for 10 days to help improve the immunity. Tab. Ursoldiolis (ursodeoxycholic acid) was used to dissolve gallstones. Conclusion: Good clinical assessment, appropriate care, good nursing care by trained nurses and appropriate treatment can save lives even in complicated COVID-19 infected cases.
