160 utaneous leishmaniasis (CL) is a skin disease which is caused by the protozoan parasites of the genus Leishmania. It causes lesions mostly in the skin and sometimes in the mucosa and heals leaving atrophic scars. 1 As it is known, a civil war started in Syria in 2011 which caused migration of a huge amount of people to neighboring countries. Approximately 3 million of these immigrants migrated to Turkey. 2 Cutaneous Leishmaniasis: Evaluation of 117 Syrian Immigrants A AB BS S T TR RA AC CT T O Ob bj je ec ct ti iv ve e: : Cutaneous leishmaniasis (CL), which presents with nodulo-ulcerative lesions in the skin, is an endemic disease in Syria and in some of the regions of Turkey. The aim of this study was to evaluate the demographic and clinical characteristics, type of treatment and treatment responses of 117 CL patients. M Ma at te er ri ia al l a an nd d M Me et th ho od ds s: : In this study, 117 Syrian CL patients, who applied to the outpatient clinic were presented. R Re es su ul lt ts s: : Sixty two of the patients were female (53%) and 55 (% 47) were male. The mean age of the patients was 16±1,4. Fifty two point one percent of the patients had one, 38.5% had two or three and 9.4% had more than three lesions. The mean duration of the lesions was 7 months, while the average duration of residence time in Turkey was 6 months. Ninety six point six percent of the lesions had a diameter less than 5 cm and the rest had larger lesions. Most of the lesions were localized on the face (43.1%) and extremities (42.8%). The majority of the lesions on the face were at the cheeks (53.8%), one of them was on the eyelid, 2 on the lip, 4 on the ear and 9 on the nose. The patients were treated with intralesional antimony compounds or cryotherapy. Lesions of the patients, who were treated with the antimony compounds recovered faster and with superior cosmetic results. Background: Clinically, cutaneous leishmaniasis (CL) can be confused with granulomatous diseases and skin cancers, and it may lead to erroneous diagnosis and treatment. Diagnosis based and histopathology can have some difficulties due to low number of parasites, especially in chronic CL cases. We aimed to emphasize the necessity of considering CL in the differential diagnosis for cases of granulomatous diseases and basal cell carcinoma, particularly in areas where CL is endemic. Methods: One hundred and seven paraffin-embedded tissue biopsy specimens were selected from the archive, as of 2002, of Pathology Department, School of Medicine, University of Hatay Mustafa Kemal in Hatay, Turkey. After DNA isolation, performed with the samples were used for PCR analysis with specific 13A, 13B primers targeting kinetoplastid DNA (kDNA) found in all Leishmania species. Another PCR was performed with LITSR and L5.8S primers targeting ITS-1 internal-transcribed-spacer-1 (ITS-1) region to subtype positive samples. Then these samples were further analyzed for subtyping with PCR-RFLP using HaeIII enzyme (BsuRI). Results: Ten out of 107 tissue specimens were positive via kDNA-PCR. Lupus vulgaris, sarcoidosis, skin lymphoma and Leishmania cutis appeared in 9 out of 10 positive specimens. One of the cases presented with a mass on the cheek and was pre-diagnosed with hemangioma, but leishmaniasis did not appear. All of 10 specimens were diagnosed as granulomatous dermatitis. Two out of 10 samples, found positive with kDNA-PCR, were analyzed with ITS-1-PCR and identified as L. infantum/donovani after RFLP. Conclusion: Molecular methods should be utilized in the differential diagnosis of CL to eliminate false diagnoses of granulomatous diseases and skin cancers.

Leishmaniasis, 98 ülkede 12 milyon insanı etkileyen vektör kaynaklı yaygın bir enfeksiyondur. Tanıda en sık kullanılan yöntem leishmania yaymaların mikroskobik incelemesidir. Bu yöntemin tanısal değeri değerlendiricinin deneyimine göre değişkenlik gösterir. Bu çalışmada kutanöz leishmaniasis tanısında lezyonal yaymaların değerlendirilmesinde deneyimin ne kadar önemli olduğunun saptanması amaçlandı. Yöntemler: Bu çalışmaya 2016 yılının Ocak ve Aralık ayları arasında Dicle Üniversitesi Tıp Fakültesi Hastanesi Deri ve Zührevi Hastalıklar Polikliniği'ne başvuran ve deri leishmaniasisi kuşkusu uyandıran lezyonlara sahip hastalar alındı. Bu hastaların tümünden hem rutin mikrobiyoloji laboratuvarında hem de kutanöz leishmaniasisin klinik ve tanısında deneyimli uzman tarafından olmak üzere ayrı yaymalar yapıldı ve birbirlerinden bağımsız olarak değerlendirildi. Bulgular: Çalışmaya alınan 98 olgunun 70'inde kutanöz leishmaniasis tanısı laboratuvar değerlendirmeleri ile doğrulandı. Kutanöz leishmaniasisin klinik ve tanısında deneyimli uzman tarafından yapılan yaymaların pozitifliği (%95,7) rutin mikrobiyoloji laboratuvarı tarafından yapılan yaymaların pozitifliğinden (%42,9) yüksekti (p<0,001). Sonuç: Çalışmamızdaki veriler gösterdi ki kutanöz leishmaniasis tanısı için yaymalar kutanöz leishmaniasisin klinik ve tanısında deneyimli uzmanlar tarafından yapılmalı ve değerlendirilmelidir.
