Clinical Practice Guideline: Ménière’s Disease

Abstract: Objective Ménière’s disease (MD) is a clinical condition defined by spontaneous vertigo attacks (each lasting 20 minutes to 12 hours) with documented low- to midfrequency sensorineural hearing loss in the affected ear before, during, or after one of the episodes of vertigo. It also presents with fluctuating aural symptoms (hearing loss, tinnitus, or ear fullness) in the affected ear. The underlying etiology of MD is not completely clear, yet it has been associated with inner ear fluid (endolymph) volume increa… Show more

“…As a result, the formerly known vestibular Meniere's disease (1,2) was placed into a category of "possible Meniere's disease" on the AAO-HNS 1995 criteria. Later definitions of Meniere's disease that were proposed by the Barany Society 2015 (4) and AAO-HNS 2000 (5) did not include the subvarieties of Meniere's disease, such as vestibular and cochlear Meniere's disease, whereas the definition by the Japan Society for Equilibrium Research (JSER) in 2017 included the vestibular type of atypical Meniere's disease (6), which is basically the same concept as that of vestibular Meniere's disease (1,2).…”

“…Moreover, the evidence from existing studies on vestibular diagnostics in MD is insufficient [22]. Additionally, the costs for both diagnostics and therapy can be very high, according to Basura et al [23]. In our opinion, the determination of SVV with the analog bucket test as an additional diagnostic tool can be justified by the presented results.…”

“…The final diagnosis in VN patients was made by caloric test (irrigation of each ear with water at a temperature of 30 and 44°C) A 20% difference calculated by Jongkees formula between the ears was accepted a pathological response. Meniere's disease (MD) diagnosis was made in accordance with the diagnostic criteria of AAO-HNS Committee on Hearing and Equilibrium Meniere's Disease [8].…”
