Clinical pharmacist interventions in post stroke follow up patients in tertiary care hospital on coastal region

Abstract: Stroke is basically a medical condition where there is low blood flow to the brain and thus results in cell death. It remains the second most common cause of death. Stroke patients also have different comorbidities on an average where they are prescribed with 6-10 medicines.
