The purpose of this study was to evaluate whether the use of desensitizing dentifrices used 15 days prior to and after in-office tooth bleaching could eliminate or reduce tooth sensitivity. After institutional review board approval and informed consent, 45 subjects were selected and divided into 3 groups according to the dentifrice selected: Colgate Total (CT), Colgate Sensitive Pro-Relief (CS) or Sensodyne ProNamel (SP). The subjects used toothpaste and a toothbrush provided to them for 15 days prior to bleaching. They were then submitted to two in-office bleaching sessions (Whiteness HP Blue Calcium). Their tooth sensitivity was assessed using the Visual Analog Scale (VAS) for a week after each session. Their tooth shade alteration was measured with a Vitapan Classical shade guide to determine if the dentifrices could influence the effectiveness of the bleaching agent. The data were submitted to Wilcoxon, Kruskal-Wallis and Mann-Whitney tests (α = 0.05). The use of desensitizing dentifrices did not affect the bleaching efficacy. In regard to tooth sensitivity, there was a statistically significant difference between the results of the Control Group and Group T2 after the first session (p = 0.048). There was no statistically significant difference in the results for the other groups after the first session. In regard to the second session, there was no statistically significant difference in the results for all the groups. The use of a desensitizing dentifrice containing nitrate potassium reduced tooth sensitivity during the bleaching regimen. Dentifrices containing arginine and calcium carbonate did not reduce tooth sensitivity. Color change was not influenced by the dentifrices used.

ÖZETAmaç: Bu in vitro çalışmanın amacı, farklı ofis tipi beyazlatma sistemlerinin beyazlatma etkinliklerinin ve renk stabilitelerinin karşılaştırılmasıdır. Gereç ve Yöntem: Seksen adet mine-dentin örneği renklenme şiddetinin standardize edilmesi amacıyla beyazlatma öncesi Orange II boyası ile renklendirildi. Örnekler rastgele dört gruba ayrıldı (n=20): (1) %40 hidrojen peroksit jeli (Opalescence Boost PF) (OB), (2) %25 hidrojen peroksit jel + ışık aktivasyonu (Zoom 2) (Z2), (3) %38 hidrojen peroksit + ışık aktivasyonu (Signal Professional White Now) (SP), (4) beyazlatma uygulaması yapılmamış grup (kontrol). Renk değerleri dental spektrofotometre ile ölçüldü. Beyazlatma sonrası dişler 9 gün boyunca kırmızı şarapta yeniden renklendirildi. Beyazlatma sonrası renk farkı (E1), yeniden renklendirme sonrası renk farkı (E2) olarak hesaplandı. Çalışma gruplarının renk farklılıklarının karşılaştırılması tek yönlü ANOVA ve Bonferonni testleri ile gerçekleştirildi ( = 0,05). Her grubun beyazlatma ve yeniden renklendirme sonrası renk farklılıkları (E1-E2) bağımlı örneklem t-testi ile analiz edildi ( = 0,05). Bulgular: Bütün beyazlatma ajanları kontrol grubuna göre daha yüksek E1 ve E2 değerleri gösterdi (p0,05), SP ve Z2 sistemleri arasında E1 ve E2 değerleri açısından fark gözlenmezken, OB ile diğer sistemlere göre daha düşük E1 ve E2 değerleri elde edildi (p0,05). Sonuç: Işık aktivasyonlu sistemlerin beyazlatma etkinliği ışık aktivasyonsuz sistemden daha yüksek bulunmuştur.Ancak bu sistemlerde yeniden renklenmeye daha fazla yatkınlık gözlenmiştir. Color values were measured using a dental spectrophotometer. After bleaching, the teeth were re-stained for 9 days using red wine. The color differences after bleaching and after re-satining were reported as E1 and E2, respectively. The color difference values of study groups were compared using one-way ANOVA and Bonferonni tests ( = 0.05). The difference between E1 and E2 values of each group were analyzed using paired samples t-test ( = 0.05). Results: All bleaching systems revealed higher E1 and E2 values than control group (p0.05). IntroduçãoAo longo das últimas décadas, há uma busca crescente por melhores condições estéticas e tratamentos clareadores, que se mostram eficazes, seguros e com bons resultados 1 . O clareamento dental é a técnica menos invasiva e mais conservadora, porém pode apresentar algumas desvantagens, como sensibilidade, ulcerações nos tecidos moles e, por vezes, toxicidade 2 . Portanto, o profissional deve ter experiência com o mecanismo de ação e conhecimento da segurança biológica dos agentes clareadores, para poder indicá-los de forma correta e orientar seus pacientes sobre o tratamento. Existem várias técni-cas de clareamento, como o clareamento caseiro com moldeira individual, o clareamento em consultório e o clareamento combinado, que associa as técnicas de consultório e caseira. As indicações e os tipos de clareamento estão relacionados às alterações de cor, que podem ser causadas por fatores extrínsecos ou intrínsecos 3,4 , e às características do paciente.Entretanto, estudos mostram que o peróxido de hidrogênio é um agente oxidante com alta capacidade de penetração no esmalte e na dentina devido ao seu baixo peso molecular 2 e, dependendo da técnica empregada, da quantidade de sessões, do tempo de exposição e da concentração do agente clareador, pode acarretar efeitos indesejáveis, como a sensibilidade e a alteração da morfologia do esmalte [5][6][7][8][9] .
