Clinical Outcomes of Conservative Treatment in Patients with Symptomatic Isolated Spontaneous Renal Artery Dissection and Comparison with Superior Mesenteric Artery Dissection

Abstract: Although the present analysis involved only a small number of patients, it revealed that symptomatic isolated SRAD with dissection related aneurysm and true lumen stenosis is a potentially life threatening condition and that aggressive surgical or endovascular interventions should be considered in these patients, who are refractory to conservative medical treatment.

“…10 Here, we show that RAD occurred in middle-aged men, frequently smokers and without any other cardiovascular risk factors. This is consistent with previous reports, in which there is a strong male predominance (from 64.7% to >90%), 3,5,8–10,12,24–27 with middle age (38.6–48 years) 3,5,8–10,12,24–27 and often smokers (58.5%–70%). 9,12,24,26,27 Preexisting hypertension is found in 6% to 27% of cases 3,5,8,12,24,26,27 but reaches 88.2% in one case series.…”

“…Dissecting aneurysm formation was defined as arterial dilatation at least 1.5 times larger than the normal mesenteric artery. 16,19 Persistent symptoms were defined as symptoms with lasting more than seven days after conservative treatment was initiated. Severe true lumen stenosis was defined as luminal narrowing of >70% of the normal mesenteric artery diameter.…”
