Objective: This study aims at evaluating the effectiveness of shoulder cuff restoration by arthroscopic surgery in dual row repair mode and shows 30 months of practical results, an ASES-based scoring system was developed. Methods: This study was conduced at mayo hospital Lahore in orthopedics department and the duration of this study was from October 2021 to march 2023. 23 patients who had significant rotator cuff tears repaired arthroscopically in a double-row fashion are included in this cross-sectional research. The operating surgeon performed a clinical assessment on patients who had been admitted for surgery in the outpatient setting both before and after surgery, as well as during follow-up visits at six weeks, three months, six months, and one year. The last examination took place on average 30 months after surgery. Using a scoring system based on a modified ASES score, the function of the patient was evaluated both preoperatively and postoperatively, and the results were compared. Based on the Modified American Shoulder and Elbow Society (ASES) score, a patient-rated questionnaire was utilized to assess shoulder and elbow function. Three parts make up the questionnaire, the first of which included questions regarding pain and had six components with a total weight of 30 points. The existence of pain, its location, and if the patients had discomfort at night were all questions that were posed to the patients. A visual analog pain scale from 0 to 10 was used to assess the degree of the patient’s pain as well as their usage of drugs. The second phase, which included 10 components and a weighted average of 50 points, examined candidates' capacity to carry out everyday tasks. On a scale of 0 to 5, patients were asked to assess their level of competence to accomplish tasks while accounting for any challenges they may have encountered. The shoulder instability was evaluated in the last portion using a visual analog scale with a range of 0 to 10. Patients were divided into four groups based on the grading system: Excellent result (score >75), Good outcome (score 50-75), Fair outcome (score 25-50), and Poor outcome (score < 25) Results: At the time of the last follow-up, there was a substantial improvement in the mean operational result scores depending on the Patient Self-Assessed Questionnaire (p-value < 0.01). By using magnetic resonance imaging to evaluate repair integrity 12 months after surgery, it was discovered that 16 patients had type-I repairs, 2 patients had type-II repairs, and only 1 patient had type-III repairs. For enormous tears, the study's retear rate was 5%. Comparing the type-III repaired shoulders to the other kinds of repaired shoulders, the type-III repaired shoulders showed substantially worse functional outcomes in terms of total scores and strength (p-value < 0.01). Practical Implication: In the practical implication of this study we discovered that the arthroscopic double-row restoration approach is a successful treatment for repairing large rotator cuff injuries with a low chance of re-tear and a big increase in activity levels. Conclusions: In this research, we discovered that the arthroscopic double-row restoration approach is a successful treatment for repairing large rotator cuff injuries with a low chance of re-tear and a big increase in activity levels. Background: It is necessary to better understand the structural characteristics of the supraspinatus tendon and associated muscle after rotator cuff repair and in the event of retear. Purpose: To study structural differences between the repaired and contralateral shoulders 1 year after rotator cuff repair in patients who received either progressive exercise therapy (PR) or usual care (UC) in a randomized controlled trial and to investigate whether there was interaction with tendon retear and limb dominance. Study Design: Cohort study; Level of evidence, 2. Methods: Patients with surgically repaired traumatic full-thickness rotator cuff tears involving the supraspinatus tendon were included. After surgery, they were randomized to PR or UC (active from postoperative week 2 or 6, respectively). The subacromial structures (acromiohumeral distance, supraspinatus tendon thickness, and vascularity) and the supraspinatus muscle thickness were examined with ultrasound at the 1-year follow-up. Results: A total of 79 patients were included. The characteristics of the 2 intervention groups (PR and UC) were comparable, including the Western Ontario Rotator Cuff Index score and number of retears. The authors found significantly thinner supraspinatus tendon (PR, P < .001; UC, P = .003) and reduced acromiohumeral distance (PR, P = .023; UC, P = .025) in the repaired versus the contralateral shoulders in both intervention groups. For neovascularization, there was no interlimb difference in either of the groups or between groups (PR vs UC). In patients with intact tendons, there was no interlimb difference in the muscle thickness, but in patients with tendon retear the muscle was significantly thinner on the repaired side ( P = .024 and P < .001, respectively). When the dominant supraspinatus tendon was repaired (both groups), it was significantly thinner than the nondominant healthy tendon, but this difference was not seen when the nondominant supraspinatus tendon was repaired ( P = .006). Conclusion: One year after rotator cuff surgery, the repaired supraspinatus tendon was significantly thinner and the corresponding acromiohumeral distance was reduced. In patients with retear, the supraspinatus muscle was significantly thinner on the repaired side and early initiation of tendon-loading exercises did not affect these findings. Introduction. Epidemiological studies demonstrate that in everyday life rotator cuff injuries are found in a wide range of population cohorts, varying from 10 to 15 % in younger adults and up to 20–30 % in patients aged 60 years and over. The significance of social and economic effects are therefore noteworthy.The objective of this study is to perform a cluster analysis and visualization of keywords in international publications on rotator cuff injuries using the VOSviewer software.Methods. The study analyzes publications related to rotator cuff injures published from 2013 to 2022 and selected from the PubMed international reference and bibliographic database for ‘Rotator Cuff Injuries’ search query. A total of 5,444 results were retrieved and relevant bibliographic information was uploaded in “.txt” format into the analytical software VOSviewer 1.6.20.Results and analysis. With the coefficient of determination (R2 = 0.96), the polynomial trend of article dynamics showed an increase in the number of publications. The average annual number of articles was (653 ± 48). With 10 repetitions, 504 keywords were analyzed and combined subsequently into 7 clusters. Cluster 1 was called ‘Trauma and injuries of the rotator cuff and tendons’. It contained 32.9 % of papers, with the total link strength of keywords 30.7 %. Cluster 2 titled ‘Results of arthroscopic reconstruction of rotator cuff injuries contained 27.7 % of papers with a total link strength of 27.5 %. Cluster 3 ‘Anatomy of the shoulder joint’ yielded 17.5 % and 17.4 % respectively; Cluster 4 ‘Biomechanics of the shoulder joint and rotator cuff’ – 7.3 % and 7.8 %; Cluster 5 ‘Postoperative analysis of surgical reconstruction of rotator cuff injuries’ – 7.1 % and 7.5 %; Cluster 6 ‘Surgical approaches (techniques) in the reconstruction of rotator cuff injuries’ – 4.6 % and 4.8 %; Cluster 7 ‘Structural reconstruction of shoulder function following reconstructive surgical of rotator cuff injuries’ – 3.9 % and 4.8 %. Leading authors and research schools were identified as well.Conclusion. The results provided an insight into the areas of research over the past 10 years and hence have provided information support for scientists and practitioners dealing with traumatic rotator cuff injuries.
