Clinical outcomes in patients who undergo extracorporeal shock wave lithotripsy for chronic calcific pancreatitis

Abstract: Extracorporeal shock wave lithotripsy fragmentation of pancreatic duct calculi in conjunction with endoscopic clearance of the main pancreatic duct is associated with significant improvement in clinical outcomes in most patients with chronic pancreatitis.

“…Large calculi need to be fragmented prior to extraction or spontaneous expulsion from the MPD. ESWL is now accepted as the standard of care in the management of large PD calculi not amenable to routine endotherapy [2,36,37,[40][41][42][43][44][45] . ESWL is very effective in fragmenting both radio-opaque and radio-lucent calculi in the MPD.…”

“…ESWL overcomes the problem of the stone size by fragmenting the stones, thus facilitating endoscopic clearance of the duct. ESWL is required in 36−68% of patients and successful rates of stone clearance of the main pancreatic duct ranged from 37% to 100% [115,120,173,[179][180][181][182][183][184][185][186][187]. Use of ultrasound instead of x-rays to locate pancreatic stones is associated with a lower fragmentation rate [188,189].…”

“…Although mechanical lithotripsy is an option, capturing an impacted stone to use this technique is rarely accomplished. Visually directed electrohydraulic and laser lithotripsy using a pancreatoscope has been more commonly utilized with the advent of single-operator pancreatoscopes [2, 17,[34][35][36][37][38]. Currently, ESWL is considered as a first-line approach in the treatment of large PD stones, impacted stones, and stones upstream to a stricture [10,39] ( Figure 21.3a-c).…”
