Clinical outcomes and safety of high-resolution manometry guided superficial partial circular muscle myotomy in per-oral endoscopic myotomy for Jackhammer esophagus: Two cases report

Abstract: BACKGROUND Jack hammer esophagus is a relatively rare disease and to date, there is no dramatic treatment option. Recently, conventional per-oral endoscopic myotomy (POEM) have expanded their area into Jackhammer esophagus. However, several complications such as post procedure motility disorders ( e.g ., passage disturbance) are issues after POEM. To overcome these issues, we here introduced high-resolution manometry (HRM)-guided superficial partial circular muscle myoto… Show more

“…This refined technique involves determining the level of esophageal hypertension via HREM, conserving the lower esophageal sphincter, and only partially removing the outer layer of the ringed muscle, rather than a full removal. Postoperatively, all symptoms were resolved within a year without complications [18]. Although POEM remains the primary endoscopic treatment for this ailment, other strategies, such as endoscopic carnitine injection and balloon dilation, have been considered [19,20].…”

“…Partial circular muscle myotomy is a technique that involves cutting only the superficial layer of the esophageal circular muscle and preserving the circular and longitudinal muscle layers. A recent report from South Korea described 2 patients with JE who were successfully treated with HREM-guided superficial partial circular muscle myotomy during POEM 100. More experience is required with this novel technique before it can be implemented routinely.…”
