Objective: To evaluate aberrometric changes in corneas with keratoconus implanted with intrastromal corneal ring segment (Keraring 355) during a 3-months follow-up. Design: Prospective, consecutive case seriesMethods: In this interventional study, 22 eyes of 21 keratoconic patients with mean age of 29 ± 6 years who underwent KeraRing 355 intrastromal corneal ring insertion using a Pocket Maker Microkeratome for channel creation and completed at least three months of follow-up were included. Uncorrected visual acuity (UCVA), best spectacle corrected visual acuity (BSCVA), refractive outcome, Orbscan finding and corneal aberrations were assessed.Results: Three months postoperately, Low order aberration (defocus and astigmatism) decreased significantly (P<0.001). No statistically significant changes were detected in total RMS, trefoil, secondary trefoil and coma. Moreover, a statistically significant increase in quatrefoil, secondary coma, spherical aberration and secondary spherical was found. In term of visual outcomes, the mean UCVA (in LogMAR value) improved significantly from 0.75 ± 0.33 to 0.31 ± 0.23 and the mean BSCVA improved (from 0.31 ± 0.16 to 0.19 ± 0.15) (P<0.05). The mean spherical refractive error improved (from -1.37 ± 1.36 to 0.01 ± 2.47) and the mean cylindrical refractive error decreased significantly (from -3.8 ± 1.03 to -2.1 ± 1.27 (P<0.001)). Also, the mean keratometry value decreased from 47.61 ± 2.03 to 42.50 ± 2.76. Conclusion: KeraRing 355 implantation provided significant improvement in low order aberrations but was not effective in high order aberrations (HOA) in low/moderate keratoconus patients. However, UCVA, BCVA, spherical and cylindrical refractive error and keratometry results improved significantly.

Цель -сравнительный анализ влияния стандартного кросслинкинга роговичного коллагена (СКРК), локального трансэпителиального фемтокросслинкинга (ЛТФ) и трансэпителиального интрастромального фемтокросслинкинга с имплантацией кольца MyoRing (ТИФ+MyoRing) на биомеханическую стабильность роговицы в эксперименте и на основе клинико-морфологических результатов у пациентов с прогрессирующим кератоконусом (КК) II и III стадии. Материал и методы. Экспериментальную часть работы выполняли на 10 кроликах (20 глаз). В клинике нами были прооперированны 48 пациентов (45 глаз) с КК II и III стадии. Животные были разделены на 4 группы по 5 глаз в каждой: 1-я группа -контроль, 2-я -глаза после СКРК, 3-я -после ЛТФ, 4-я -после ТИФ+MyoRing. Фемто-этап выполняли с помощью фемтосекундного лазера IntraLase FS 60 кГц, этап кросслинкинга -с применением аппарата Evolution. Срок наблюдения составил 1 мес. Все пациенты в зависимости от метода операции были разделены на 3 группы. В 1-й группе 9 пациентам (14 глаз) проведена ТИФ+MyoRing, во 2-й группе 20 пациентам (16 глаз) выполнена процедура СКРК, в 3-й группе 19 пациентам (15 глаз) -процедура ЛТФ. Срок наблюдения составил 6 мес. Результаты. Результаты эксперимента показали значительное увеличение силы, необходимой для разрыва роговичных образцов, во всех трех группах. У пациентов 1-й группы через 6 мес после операции было отмечено увеличение фактора резистентности роговицы (ФРР) на 1,0±0,22 мм рт.ст, корнеального гистерезиса (КГ) -на 1,0±0,12 мм рт.ст, во 2-й группе ФРР увеличился на 0,8±0,10 мм рт.ст, КГ -на 0,6±0,16 мм рт.ст, в 3-й группе ФРР повысился на 0,8±0,25 мм рт.ст, КГ -на 0,6±0,26 мм рт.ст. Во всех трех группах не было отмечено в течение всего периода наблюдения статистически значимого снижения плотности эндотелиальных клеток. Заключение. Все три метода -ЛТФ, ТИФ и СКРК в равной степени увеличивают биомеханическую прочность роговицы и являются безопасными. ТИФ+MyoRing у пациентов с прогрессирующим КК II и III стадии при сроке наблюдения 6 мес демонстрирует более высокую биомеханическую стабильность роговицы за счет сочетания механического и химического методов ее укрепления.Ключевые слова: кросслинкинг роговичного коллагена, фемтосекундный лазер, интрастромальный фемтокросслинкинг, кольцо MyoRing, кератоконус.
