Background: Respiratory distress syndrome (RDS) is the single most important cause of morbidity and mortality in preterm neonates. Early management of RDS is crucial in determining short- and long-term outcomes and studies have established initial respiratory support (IRS) among other factors as an important determinant. Despite preexisting guidelines and advancements in the management of RDS, IRS failure with noninvasive ventilation is common and is associated with unfavorable outcomes. Objective: This study evaluated the non-invasive initial respiratory support outcomes and associated factors among preterm neonates with respiratory distress syndrome admitted in the newborn unit (NBU) at Moi Teaching and Referral Hospital Eldoret (MTRH), Kenya Methods: Using a prospective observational hospital-based study, preterm neonates ≤ 35weeks admitted at the newborn unit with clinical RDS were followed up for 28 days. The primary outcome was IRS success or failure characterized by not stepping up or stepping up the respiratory support respectively within 72 hours of life and associated maternal and neonatal factors. Descriptive statistics was described using mean ± (SD) for continuous variables and frequencies and percentages for categorical variables. Simple and multinomial regression analysis was performed to evaluate relationship between different IRS methods with outcome variables and a p-value of < 0.05 was considered significant. Results: We enrolled 320 neonates, 172(53.8%) were male with a mean (SD) gestation age of 30.9 (2.95) weeks. The mothers mean age was 27 years, ranging (15-43). 70(22.4%) 95%CI:17.95,27.47] had IRS failure and 243(77.6%) had IRS success. On multivariate analysis IRS success was associated with primiparity (AOR=2.81;95%CI: 1.42, 7.99), birthweight > 1300g (AOR= 5.04;95%CI 1.81, 14.6), low modified Downes score (AOR=26.395%CI 3.37, 230) and normal admission temperatures (AOR=0.32;95%CI 0.12, 0.72) (p= <0.001). Conclusion: Noninvasive ventilation had a high initial respiratory support success. Primiparity, birthweight >1300g, normal admission temperatures and low Downes score were associated with IRS success.

Background: Neonatal respiratory distress syndrome is a respiratory problem recognized as one or more signs of tachypnea, Bradypnea, nasal flaring, chest retractions, grunting, and respiratory pauses. It is one of the leading causes of neonatal mortality, especially in developing countries. Although it is one of the leading causes, there haven't been studies conducted on mortality rates and predictors in Ethiopia. Objective: To assess the mortality rate and its predictors among neonates with respiratory distress syndrome admitted to the neonatal intensive care unit of Debre Markos comprehensive specialized hospital, Northwest Ethiopia. Method: An institution-based retrospective cohort study was conducted among 384 neonates admitted with respiratory distress syndrome to the neonatal intensive care unit of Debre Markos comprehensive specialized hospital from January 1, 2020, to March 30, 2023. Data was extracted using checklist and entered into EPI Data 4.6. Finally, export to STATA version 14.1 for further management and analysis. The Kaplan-Meier curve with the log-rank test was computed to estimate failure time and compare failure status between independent groups. Both bivariable and multivariable Cox regression models were applied to identify predictors of mortality. The model assumption was checked by a graph and the Schoenfeld residual global test. The final model's fitness was checked using the Cox-Snell residual test. Result: Among 384 neonates with RDS 36.7% were dead during t study period, with an overall mortality rate of 7.5 per 100 neonate days of observation. The median time to mortality was 13 neonatal days (3 and 16 days of IQR), with a total follow-up of 1864 neonate-day observations. Preterm (AHR=1.54, 95% CI: 1.03–2.31), home delivery (AHR=2.25, 95% CI: 1.28–3.95), and maternal hypertension (AHR=3.59, 95% CI: 1.66-7.77) were the significant predictors. Conclusion: This study found that the neonatal mortality rate is high. The predictors significantly associated with neonatal mortality among neonates admitted with RDS were preterm birth, home delivery, and neonates born from hypertensive mothers. Collaboration efforts are needed with families, labor and NICU health care providers, institutions, and the community to reduce neonatal mortality.

PurposeRespiratory distress syndrome (RDS) is potentially fatal in infants. The present study investigated the association of maternal serum vitamin D level with fetal pulmonary artery Doppler indices and neonatal RDS.MethodsThis cross‐sectional‐analytical study was conducted on 260 mothers admitted for delivery. The maternal serum 25‐hydroxyvitamin D level was measured and fetal main pulmonary artery Doppler indices were evaluated. The neonate's weight, apgar score, RDS, Umbilical cord arterial pH and neonatal intensive care unit admission were recorded.ResultsThere was a significant relationship between RDS and the low level of vitamin D in the mother's serum. Mothers with low vitamin D had higher fetal pulmonary artery pulsatility index (PI) and peak systolic velocity (PSV) and lower acceleration time (AT) to ejection time (ET) ratio. Also, in neonates with RDS, pulmonary artery PI was higher, and PSV and the ratio of AT to ET were significantly lower than neonates without RDS. Neonates with lower one and 5‐min apgar scores were born from mothers with lower vitamin D levels.ConclusionSerum levels of vitamin D in pregnant mothers as well as fetal pulmonary artery Doppler indices are correlated to RDS. There is a significant relationship between the serum level of vitamin D in pregnant mothers and fetal pulmonary artery Doppler indices.
