Clinical impact of strain histogram EUS elastography and contrast-enhanced EUS for the differential diagnosis of focal pancreatic masses: A prospective multicentric study

Abstract: Background: Recent advances in EUS techniques (real-time EUS elastography and contrast-enhanced EUS) have allowed a better characterization of focal pancreatic masses. Mean strain histograms (SHs) are considered a good parameter for the semi-quantitative evaluation of focal pancreatic masses, alongside complementary contrast-enhanced EUS parameters which can be quantified during both the early arterial and late venous phase. Materials and Methods: The study design was prospective, blinded, and multicentric, as… Show more

“…Probably, the role of ancillary techniques such as elastography and use of EUS contrast agents and/or the use of software of automated image capture and analysis with use of artificial intelligence, will become increasingly important and will contribute to define the clinical behavior of PanNENs preoperatively. [ 35 36 37 ] The present data suggest that PanNENs grading can be correctly evaluated preoperatively by EUS-FNA cytology in some two thirds of patients. In this view, the concomitant use of additional techniques as PET with gallium and FDG may be important to support clinical choices.…”

“…Such hyperenhancement can be helpful to distinguish focal autoimmune pancreatitis and pancreatic cancer since inflammatory diseases usually show some degree of enhancement [93][94][95]. CE-EUS combined with elastography plays a fundamental role in complementary diagnostic of focal pancreatic masses [96]. However, due to a specificity lower than 90%, the authors recommend to combine imaging and biopsy techniques for a correct diagnosis.…”

“…Another method is represented by the strain histogram, which is based on the hue histograms averaged over several compression cycles [18,19]. The results are usually described by mean values and standard deviation, as well as other parameters characteristic to histograms (Kurtosis, skewness, etc.).…”
