Clinical impact of functional independent measure (FIM) on 180-day readmission and mortality in elderly patients hospitalized with acute decompensated heart failure

Abstract: Activities of daily living (ADL) are important prognostic factors for heart failure. The functional independent measure (FIM) has emerged as a comprehensive valid measure of ADL from both physical and cognitive perspectives. This study aimed to investigate the prognostic impact of the FIM score on clinical outcomes in hospitalized patients with acute decompensated heart failure (ADHF). We retrospectively analyzed 473 ADHF patients, with available pre-discharge FIM scores, admitted to our institution between Ma… Show more

“…It is clear that Japanese patients with HF are at a high risk of readmission within 1 year 13) . The readmission rates in previous studies were 5.4% within 30 days, approximately 20% within 90 days, approximately 24% within 180 days, and 26%-35% within 1 year; and the risk of readmission increased with increasing observation period 10,11,22,36) . Therefore, continued follow-up after preventing early readmission is important.…”

“…Our data may be important for preventing readmission among older adults with HF. Reports of early readmission (90-180 days) after discharge have increased over the past few years 22,24,36) , but very few studies have followed patients for longer periods. It is clear that Japanese patients with HF are at a high risk of readmission within 1 year 13) .…”

“…The primary outcome was the FIM-motor score at discharge, which was evaluated by trained rehabilitation therapists [ 45 , 46 ]. The FIM is divided into a motor domain (FIM-motor) with 13 sub-items and a cognitive domain (FIM-cognitive) with 5 sub-items [ 38 ].…”

“…In recent years, there has been a great deal of clinical and research interest in sarcopenia and frailty as a means of evaluating prediction of the event [1][2][3][4]. As for evaluation of sarcopenia and frailty, many theories have been proposed to relate them to various evaluation methods [5,6]. Asian Working Group for Sarcopenia (AWGS) has also focused on handgrip strength and has identified lifestyle interventions [7], particularly exercise and nutrition, as the mainstay of treatment and prevention to improve low muscle strength [8,9].…”
