Clinical, Functional and Pulmonary Hemodynamic Course of Patients with Chronic Obstructive Pulmonary Disease Followed-Up Over 3 Years

Abstract: In 65 subjects with chronic obstructive pulmonary disease, mainly chronic bronchitics, all exhibiting arterial hypoxemia and moderate to severe airways obstruction (FEV1 ranging from 625 to 1,980 ml), the following data were regularly followed-up over 3 years: clinical status, chest X-rays, ECG, arterial blood gases, pulmonary volumes, pulmonary hemodynamics (at least two catheterizations were performed). Strasbourg 67000 (France The follow-up period defined by the time elapsed between the first and… Show more

“…The correlation between FEV1 and Ppa is controversial; in one study, no correlation was found [23], whilst another long-term study reported a weak inverse correlation [16]. In the present study, in accordance with these results, a significant inverse correlation was found between Pa,O 2 and Ppa, but no correlation between FEV1 and Ppa.…”

“…Pulmonary hypertension is a well-known complication of severe lung diseases such as chronic obstructive pulmonary disease and idiopathic pulmonary fibrosis (1)(2)(3)(4)(5)(6). Pulmonary hypertension secondary to chronic lung diseases is usually moderate, rarely exceeding 35 to 40 mm Hg, and is related to alterations in blood gases, abnormal pulmonary mechanics, and relatively subtle vascular remodeling (5).…”

“…The present study was therefore designed to extend this observation. Accordingly, the objectives of this study were as follows: ( 1 ) to evaluate the pulmonary hemodynamics in consecutive patients with pulmonary histiocytosis X and other chronic lung diseases referred to our center for lung transplantation, and ( 2 ) to correlate pulmonary hemodynamics with respiratory function tests and pulmonary vessel histopathology.…”

“…Thus, the 2-yr mortality in our R is considerably lower than the lowest mortality reported for patients not receiving LTD0 2 . It is also lower than the reported mortality for patients receiving LTD0 2 , even in those who had the best prognostic indicators in different studies (6,10,11). On the other hand, the mortality in our NR is no better than in patients not treated with LTD0 2 (7,20,(22)(23)(24).…”

“…However, a number of patients show no improvement with oxygen therapy. Identification of these patients would spare the expense, inconvenience, and hazards (8,9) of long-term oxygen therapy, and it also might turn the attention of the physician to potentially more effective measures, such as the control of heart failure and infection (10), vasodilators (11), dextran infusion in patients with polycythemia (12), and a rehabilitation program (13). The importance of predicting a good response to oxygen therapy has been widely acknowledged (13,14).…”
