Clinical Experience With Primary Percutaneous Transluminal Coronary Angioplasty Compared With Alteplase (Recombinant Tissue-Type Plasminogen Activator) in Patients With Acute Myocardial Infarction

Abstract: These findings suggest that in lytic-eligible patients not in shock, PTCA and rt-PA are comparable alternative methods of reperfusion when analyzed in terms of in-hospital mortality, mortality plus nonfatal stroke and reinfarction.

“…Two of 123 patients without cardiogenic shock died (1.6%) which is similar to international data which shows in-hospital mortality of 5.2% in the second national registry of myocardial infarction (NRMI2) and 3% in ASSENT 4 trial [17]. In our study 16 patients had cardiogenic shock and four of them died (25%) which is similar to available data-32% in NRMI 2, 46.4% in SHOCK registry and 59.1% in American College of CardiologyNational Cardiovascular Data Registry (ACC-NCDR) [18,19].…”

“…Drug eluting stents was used in 87% while only 7 patients received baremetal stents this was due to large culprit vessel and high bleeding risk. In hospital mortality was 5.1% comparable to international rates which showed in hospital mortality of 5.2% in second national registry of myocardial infarction (NRM-12) and 3% in ASSENT 4trial (18,19).…”

“…19 Most patients with cardiogenic shock do not undergo emergency revascularization, either because of the lack of facilities at the hospitals where they present or because of doubt as to its efficacy. 18,[20][21][22] If early revascularization reduces mortality, angioplasty or coronary-artery bypass graft surgery should be performed on an urgent basis, even if this requires transfer to a hospital with the necessary facilities and expertise.…”
