Clinical Evaluation Comparing the Efficacy of Aquacel Ag with Vaseline Gauze versus 1% Silver Sulfadiazine Cream in Toxic Epidermal Necrolysis

Abstract: This study showed that Aquacel Ag with Vaseline gauze decreased pain and labor costs but did not shorten wound healing time. Thus, Aquacel Ag with Vaseline gauze can be an efficient method for treating TEN wounds.

“…7 In contrast, case reports associated systemic silver absorption with subsequent argyria, elevated transaminases, and leukopenia. 2,4 Our study indicates that silver nylon and silver foam dressings may lead to systemic silver absorption, similar to other silver compounds. Our study also highlights the need of larger research studies to investigate whether the observed leukopenia and elevated transaminases are a result of silver toxicity, an underlying haematological malignancy (as noted in 3 of our patients), or to SJS/TEN itself.…”

“…Recent publications have demonstrated a trend towards the use of modern dressings for wound care in SJS/TEN, including silver-impregnated dressings, for the antimicrobial properties and improved patient comfort secondary to decreased frequency of dressing changes. [1][2][3][4][5] However, cytotoxic effects of silver have been demonstrated in human cell lines, including fibroblasts, keratinocytes, granulocytes, monocytes, and macrophages. 6,7 Animal and human studies addressing the possibility of silver toxicity from silvercoated dressings applied to burns have yielded conflicting results with regard to systemic silver absorption.…”

“…However, the silicone tube itself has poor plasticity, which makes it easy to produce joint space during the padding process. Therefore, the padding process requires the use of Vaseline-coated gauze, which is more difficult to work with [13]. Chitosan, a new natural macromolecule material that is non-toxic, biocompatible, and biodegradable, is used in clinical wound dressing to promote wound healing and prevent postoperative adhesions.…”

“…The application of sterile Vaseline permeated gauze maintains a moist wound surface to facilitate healing. 16,17 In this study, essential Vaseline album used for ointment only served to moisturize the surface of the skin while Adeps played its role to absorb the liquid. However, since it does not contain any active ingredient, the restoration of tissue in the wound that was only given the base of the ointment was not disrupted so that the healing phases such as inflammation, re-epithelization and connective tissue formation can be processed naturally to recover itself, as long as there is no infection of the wound.…”
