ObjectivePatients with structural abnormalities of cardiac valves, including bicuspid aortic valve (BAV), are said to be at higher risk of infective endocarditis (IE). We sought to determine the risk of IE of the BAV compared with the tricuspid aortic valve (TAV) and to determine the risk of aortic valve replacement and mortality after IE.MethodsFrom medical records of two US and one Italian hospitals, patients with their first episode of IE of any native valve were identified. In the US cohort 42 patients with BAV and 393 patients with TAV with IE occurring between 1 January 2000 and 30 June 2014 were identified. In the Italian cohort 48 patients with BAV and 341 patients with TAV with IE underwent valve replacement surgery between 1 January 2000 and1 November 2015. The risk of IE for BAV and TAV and subsequent outcomes were determined after matching to patients without IE.ResultsAfter adjustment for risk factors, the risk of IE in the US cohort was 23.1 (95% CI 8.1 to 100, p <0.0001) times greater for BAV than TAV. Patients with BAV with IE were more likely to have an aortic root abscess. Within the subsequent 5 years, BAV patients with IE were more likely to undergo valve replacement (85%) than TAV patients with IE (46%). Patients with IE were at increased risk of death. The findings were similar in the Italian cohort.ConclusionsPatients with BAV are at markedly increased risk of IE and aortic root abscess than patients with TAV. Increased risk of IE in patients with BAV indicates they may be a candidate group for long-term trials of antibiotic prophylaxis of IE.

Introduction: Infective endocarditis (IE) is a serious disease with a high mortality rate and is defned as an inﬂammation of the endocardium, caused by microorganisms. Most data about IE originate from referral hospitals, therefore we aimed to analyse the demographics, presentation, diagnostics, treatment and outcome of IE at a regional hospital (Izola General Hospital–SBI) in a 10-year period.Methods: We analysed 51 episodes of IE and compared patients who were treated at a regional hospital alone, to those transferred to a referral centre. We also included data about all IE patients in a large referral hospital (University Medical Centre Ljubljana–UMCL).Results: Gender and age of the patients in SBI were similar to UMCL patients, however, there was a slightly higher share of IE in drug users and lower of prosthetic valve endocarditis. Our patients more often had streptococcal and less often staphylococcal endocarditis. Fewer patients needed surgery and time to operation was 17 days (median). Twenty-six percent of the patients died in the hospital. Approximately a half of the patients were transferred to a referral hospital, more often younger patients and women. The transferred patients often underwent surgery.Conclusions: Good cooperation with a referral hospital ensures that the outcome and other data of IE patients treated at a regional hospital are comparable to those in the literature. Some diﬀerences between patients treated in the regional hospital and the referral centre were observed. Time from admission to surgery could be shortened.

В представленном обзоре отмечено изменение эпидемиологического про-филя инфекционного эндокардита (ИЭ) с увеличением заболеваемости в пожилом возрасте. Обсуждаются факторы риска ИЭ, включая перенесен-ные ранее заболевания эндокарда различного генеза, имплантацию искус-ственного клапана, проведение различных лечебных и диагностических инвазивных процедур и особенно кардиохирургических вмешательств. Среди факторов риска, предрасполагающих к инфекционному поражению эндокарда у пожилых лиц, отмечены кальцификация клапанов, возрастные изменения эндокарда, наличие сахарного диабета, воспалительных забо-леваний желудочно-кишечного и урогенитального тракта. приведены осо-бенности этиологического спектра возбудителей ИЭ, характеризующиеся увеличением частоты случаев, вызванных S. bovis, энтерококками, коагула-зонегативными стафилококками по сравнению с более молодыми группами больных. В обзоре особое внимание уделяется микробиологической диа-гностике, как одному из больших критериев в диагностике ИЭ. подходы к лечению заболевания приведены с учетом общих принципов, выбора эмпирической терапии и назначения соответствующих антибактериальных препаратов после получения результатов бактериологического исследова-ния гемокультуры. принципы лечения пожилых больных с ИЭ основаны на рекомендациях экспертов Европейского Общества кардиологов и Аме-риканской Ассоциации Сердца, опубликованных в 2015г. подчеркивается важность профилактики инфекционного поражения эндокарда у лиц высо-кого риска при применении манипуляций высокого риска, а также соблюде-ния общих гигиенических мер и санитарных норм.
