Introducción: La pérdida prematura de uno o más dientes temporales en niños menores de 10 años dada principalmente por trauma dental o acción cariogénica ocasiona la reducción del espacio de las piezas definitivas, donde se vuelve fundamental el empleo de mantenedores de espacio acorde a las necesidades del caso, ya que la elección de un método adecuado evitaría problemas posteriores tanto por la pérdida del espacio como por un uso inadecuado de estos dispositivos. Objetivo: Analizar el empleo de mantenedores fijos en ausencia de molares temporales de pacientes menores a 10 años. Materiales y métodos: Se realizó una revisión descriptiva de la literatura con empleo de estrategias de búsqueda en la base de datos PubMed publicados entre 2017 al 2022. Resultados: Con las estrategias utilizadas se obtuvieron 53 artículos, de los cuales se eliminaron los que no contenían comparaciones entre mantenedores de espacio y fallas en la cementación quedando 14 artículos que fueron revisado y expuestos a seguir. Se hace necesario la generación de estudios comparativos y a largo plazo para una estandarización de los protocolos de tratamiento, mientras tanto la elección del método para mantener el espacio y el control periódico son fundamentales para el éxito de este. Conclusión: Podemos concluir que los mantenedores de espacio fijos son los ideales en pacientes menores a 10 años con pérdida de molares temporales ya que inevitablemente se pierde el espacio de la pieza faltante y en la mayoría de casos conlleva a problemas posteriores.

The primary goal of orthodontic therapy in pseudo-class III is to restore the proper dental connection by rectifying the canine and molar relationship to Class I through lower molar and premolar visualization, as well as providing normal anterior overjet. The purpose of this systematic study was to determine the efficacy of clear aligners in treating class III malocclusion with mandibular molar distalization. A wide range of searches were done on various search engines like Cochrane, Web of Science, Embase, PubMed, Scopus, and Google Scholar to collect relevant articles related to our study. This review's article selection was guided by the PRISMA flowchart. The electronic findings provided numerous articles with nearly 78 articles regarding clear aligners in class III malocclusion with molar distalization. From this, seven full-text papers were evaluated for eligibility criteria, with two articles being rejected with justification and five articles being elaborated in the current systematic review. The current evidence of this review suggested that the clear aligners were effective in correcting class III malocclusion with molar distalization. The amount of molar distalization is about 2 to 3 mm, which helps in achieving molar and canine relationship in class I, with a high compliance level and also improvement of the facial profile.

Background: Lower molar distalization is considered to be one of the most difficult movements to obtain with clear aligners. The aim of this study was to evaluate the possibility to distalize the lower molars using clear aligners in adults without miniscrews. Material and methods: rx cephalograms of 16 patients (8 males, 8 females; mean age of 25.6 years), who underwent lower molar distalization, were analyzed. Cephalograms were taken before the beginning of treatment (T0) and after the end of treatment (T1). Dental and skeletal changes between T0 and T1 were evaluated. The level of significance was set at p < 0.05. Results: At T1, the lower second molar moved distally 2.47 mm, but there was a significant tipping (p = 0.027); the same result was found at the first molar, with a mean distal movement of 1.16 mm and a significant tipping (p = 0.003). No significant changes were detected on the sagittal and vertical skeletal variables. Conclusion: Clear aligner therapy can provide more distal tipping than bodily movement in the lower molars when a distalization is programmed. This movement could be sufficient to correct the initial malocclusion and gain a class I molar relationship.
