Background The Appalachian population is distinct, not just culturally and geographically but also in its health care needs, facing the most health care disparities in the United States. To meet these unique demands, Appalachian medical centers need an arsenal of analytics and data science tools with the foundation of a centralized data warehouse to transform health care data into actionable clinical interventions. However, this is an especially challenging task given the fragmented state of medical data within Appalachia and the need for integration of other types of data such as environmental, social, and economic with medical data. Objective This paper aims to present the structure and process of the development of an integrated platform at a midlevel Appalachian academic medical center along with its initial uses. Methods The Appalachian Informatics Platform was developed by the Appalachian Clinical and Translational Science Institute’s Division of Clinical Informatics and consists of 4 major components: a centralized clinical data warehouse, modeling (statistical and machine learning), visualization, and model evaluation. Data from different clinical systems, billing systems, and state- or national-level data sets were integrated into a centralized data warehouse. The platform supports research efforts by enabling curation and analysis of data using the different components, as appropriate. Results The Appalachian Informatics Platform is functional and has supported several research efforts since its implementation for a variety of purposes, such as increasing knowledge of the pathophysiology of diseases, risk identification, risk prediction, and health care resource utilization research and estimation of the economic impact of diseases. Conclusions The platform provides an inexpensive yet seamless way to translate clinical and translational research ideas into clinical applications for regions similar to Appalachia that have limited resources and a largely rural population.

This article proceeds with a discussion of the causes and mechanisms of an elevation in cardiac troponins in pathological conditions not associated with acute myocardial infarction. The second part of the article discusses the causes and mechanisms of cardiac troponins elevation in diabetes mellitus, arterial hypertension, hereditary cardiomyopathies, cardiac arrhythmias (atrial fibrillation, supraventricular tachycardia), acute aortic dissection, and diseases of the central nervous system (strokes, subarachnoidal hemorrhage). The final chapter of this article discusses in detail the false-positive causes and mechanisms of elevated cardiac troponins.

На фоне улучшения чувствительности лабораторных методов детекции сердечных тропонинов (cTn) открылись новые диагностические возможности для их применения в клинической практике. Новые высокочувствительные методы определения тропонинов (hs-cTn) позволяют выявлять даже самые незначительные повреждения кардиомиоцитов при многих сердечных и системных заболеваниях, и степень повышения hs-cTn, как правило, имеет значительную диагностическую и/или прогностическую ценность. Многие из таких заболеваний, в частности артериальная гипертензия (АГ), могут протекать латентно и в последующем часто приводить к возникновению ряда опасных и тяжелых осложнений в виде острого инфаркта миокарда, сердечной недостаточности, инсульта и др. Поэтому раннее выявление пациентов с АГ, имеющих повышенный риск возникновения данных осложнений, будет иметь большое значение для улучшения лечебно-профилактических мероприятий в клинической практике. В данной статье подробно обсуждаются возможность использования и диагностическое значение повышенных уровней hs-cTn в биологических жидкостях у пациентов, страдающих АГ. Отдельное внимание уделено механизмам повышения hs-cTn при АГ в биологических жидкостях человека. Against the background of improving the sensitivity of laboratory methods for detecting cardiac troponins (cTn), new diagnostic opportunities have opened up for their use in clinical practice. New highly sensitive methods for determining troponins (hs-cTn) make it possible to detect even the most insignificant damage to cardiomyocytes in many cardiac and systemic diseases, and the degree of increase in hs-cTn, as a rule, has significant diagnostic and/or prognostic value. Many of these diseases, in particular arterial hypertension (AH), can occur latently and subsequently often lead to a number of dangerous and severe complications in the form of acute myocardial infarction, heart failure, stroke and others. Therefore, earlier identification of patients with hypertension who have an increased risk of these complications will be important for improving therapeutic and preventive measures in clinical practice. This article discusses in detail the possibility of using and the diagnostic value of elevated levels of hs-cTn in biological fluids in patients suffering from AH. Special attention is paid to the mechanisms of increasing hs-cTn in hypertension in human biological fluids.
