Introduction: Although the liver and lung are the most frequently affected organs in cystic echinococcosis, the cysts may develop in any viscera and tissues. Breast is a rare primary localization with few cases described in the literature. We present an updated and systematic review and discuss the possible mechanisms of spreading, diagnostic and treatment options.Materials and methods: We performed a literature search in PUBMED using the key words ‘hydatid disease’, ‘cystic echinococcosis’ and ‘breast echinococcosis’ without time limitation. Only studies reporting breast cystic echinococcosis were included.Results: Overall, 121 cases with cystic echinococcosis and 2 with alveolar echinococcosis were reported. A total of 52 cases were included in the analysis. The mean size of cysts was 5.5 cm (range 1.7-12). The most common clinical presentation was painless lump presented from 4 months to 19 years before the final diagnosis. Most cases had isolated breast CE, few cases had synchronous localizations – femoral, thigh and lung, and previous liver CE. Most were active CL and CE1-2 cysts (72%). Ultrasound was used in 83%, followed by mammography (35%). Fine needle aspiration was reported in 27 cases with positive finding in 59%.Conclusions: In cases with cystic breast lesions from endemic regions we recommend the US as a gold standard. CT and MRT are more accurate but expensive tools without the potential to change the surgical tactic. In contrast to the other localizations of CE, complete excision of the cysts is the best diagnostic and treatment approach.

Only one case of thyroid HC was noted in the Akcam AT, et al [12]. Turkish series of 661 operated HC including 134 of unusual localization (0.15%) and a single case was also noted in Çakir M, et al [8] series of 157 HC of unusual localization (0.6%).…”

Introduction:The thyroid localization of the hydatid cyst (HC) remains exceptional and unusual even in countries where this Parasitosis is still endemic: its frequency varies from 0.1 to 0.6 ‰ depending on the series. It is most often asymptomatic or resulting in anterior cervical swelling or more rarely a hoarseness of the voice. We report an original observation of thyroid HC revealed by primary hypothyroidism.Observation: An eight-year-old patient with no notable pathological history was explored for normochromic normocytic anemia at 8 g/dl. The clinical examination found neither jaundice nor splenomegaly. There was anterior cervical swelling that was painless and lateralized to the right. No lymphadenopathy or other palpable mass was noted. Biology revealed peripheral hypothyroidism with TSH at 7.83 μIU/ml. The balance of hereditary hemolytic anemias was negative. Cervical ultrasonography and CT showed an anechoic anterior cervical mass 2.51 cm in diameter, occupying the right thyroid lobe, without micro calcifications, intracystic vegetation, or adenopathies. Hydatid serodiagnostic was positive. The diagnosis of thyroid HC was confirmed by histological examination post loboishmectomy. The total blood count and TSH were spontaneously corrected after one month of the surgery. Conclusion:The diagnosis of HC is worthy of mention in the presence of peripheral hypothyroidism with increased thyroid gland volume, especially in endemic areas. This location can sometimes hide associated thyroid neoplasia.

Objectives:To compare results of surgical treatment and complications of patients with unilateral or bilateral thoracic and combined pulmonary echinococcosis.Methods:This cross-sectional analysis of a prospective study was conducted in the Department of Thoracic and Pediatric Surgery, Scientific Center of Surgery, Almaty, Kazakhstan among 598 patients with pulmonary echinococcosis, who had surgical treatment with various surgical methods, depending on the prevalence of echinococcosis, as follows: right lung in 357 (59.5%) patients, left lung in 243 (40.5%) patients, bilateral in 95 (15.8%) patients, and complicated echinococcosis in 317 (52.8%) patients. Length of stay per hospital stay has been decreased (p<0.0001) by video-thoracoscopic echinococcectomy with the high-energy laser (HEL) treatment of cyst, than after echinococcectomy by cyst treatment with povidone-iodine. Treatment with formalin presented the most longest hospital stay (p<0.0001)Results:Comparative analysis of patients with uncomplicated and complicated pulmonary echinococcosis showed a high frequency of postoperative complications associated with complicated echinococcosis (OR = 2.2, p<0.0001).Conclusion:Despite the success of surgical treatment of pulmonary echinococcosis, issues of intraoperative dissemination and safety remain, and treatment success rates can be improved. These factors require further prospective multicenter studies.
