Lymphangioma is a benign congenital malformation of the lymphatic tissue that occurs predominantly in children and clinically presents as a soft painless swelling, compressible of oral mucosa. Usually affects male individuals and the most common intraoral location is the tongue. Surgical excision is the treatment of choice and the prognosis is good. It is describes a case of lymphangioma in a non-caucasian 82 years old female. Intra-oral examination revealed the presence of reddish papules, with 0.5 cm in diameter, painless, flaccid consistency and smooth surface in the anterior hard palate. Excisional biopsy was performed and the histopathologic diagnosis was lymphangioma. After seven months follow-up, the patient has no recurrence of the lesion.

Résumé -Introduction : Les malformations lymphatiques (ML) sont congénitales et non régressives. Elles résul-tent d'une anomalie de la morphogenèse embryologique vasculaire et sont détectées dans 80 % des cas avant l'âge de 2 ans, exceptionnellement à l'âge adulte. Asymptomatiques, elles sont le plus souvent diagnostiquées suite à une gêne fonctionnelle. Bien que d'aspects cliniques très hétérogènes, elles se regroupent en deux formes cliniques à distinguer, macrokystiques et microkystiques, dont les traitements et les pronostics diffèrent. L'aspect clinique de la lésion et la symptomatologie sont essentiels pour l'établissement du diagnostic mais aussi pour la décision thérapeutique. Bien qu'une surveillance clinique régulière soit souvent suffisante, une intervention thérapeutique est parfois nécessaire. Observations cliniques : Quatre cas de ML orales avec des aspects cliniques très hétéro-gènes sont présentés. L'un est situé au niveau de l'espace masticateur et de la fosse infratemporale avec une expression muqueuse à la commissure intermaxillaire, deux au niveau des muqueuses jugales et labiales, le quatrième au niveau de la langue. Discussion : Pour trois d'entre eux, le diagnostic a été réalisé à l'âge adulte et ceuxci ne présentaient aucun antécédent de ML. Deux ont été traités par laser, un par chirurgie, le dernier a bénéficié d'une surveillance clinique régulière. Conclusion : Ces quatre cas cliniques sont particulièrement représentatifs de l'hétérogénéité clinique buccale et thérapeutique de ces malformations.Abstract -Oral lymphatic malformations, case reports and review of the literature. Introduction: Lymphatic malformations (LMs) are congenital and non-regressive lesions. These lesions are due to embryological vascular morphogenesis abnormality. Most of them (80 %) are detected before 2 years old, and thus they are rarely found in adults. They are asymptomatic, so their diagnosis is often linked to functional discomfort. There are two clinical aspects with different prognoses: macrocystic and microcystic. Clinical aspects and symptomatology are essential for positive diagnosis and therapeutic decisions. For most cases, simple clinical observation is advised. However, in other cases associated with aesthetic or functional discomfort, therapeutic intervention is recommended, most of the time with sclerotherapy or surgery. Clinical observation: We present four patients with different types of lymphatic malformation: one is on the intermaxillary commissure, the second is on the cheek mucosa, the third is on the lip and the fourth is on the tongue. Discussion: For three of them, the diagnosis was established in adulthood with no past history of LMs. Two patients were treated by laser, one with surgery and for the last one, clinical follow-up was established. Conclusion: These four case reports are representative oral illustrations of the clinical aspects and therapeutic heterogeneity of this pathology.

Oral lymphatic malformations: A multicenter study of 208 cases and literature review

Background To evaluate the frequency and analyze demographic and clinical characteristics of individuals with a histopathological diagnosis of oral lymphatic malformations (OLMs). Methods A multicenter study was performed, collecting biopsy record data from a consortium of Brazilian Oral and Maxillofacial Pathology Centers. A review was also conducted to compare this data with cases already available in the literature. Results This study retrieved 208 cases of OLM in the multicenter study and 1035 cases in the literature review. In both, OLMs affected male and female individuals equally, with the most affected site being the tongue. Individuals ≥60 years of age were uncommonly affected. Symptomatic and larger lesions were more commonly reported in the literature review. Conclusions This study comprises the largest sample of OLMs to date. OLMs are rare conditions, without sex predilection. The elderly proved to be less frequently affected, and the tongue is the most commonly affected site.
