Clinical characteristics and outcomes of acute coronary syndrome patients with left anterior hemiblock

Abstract: Across the broad spectrum of ACS, LAHB was associated with significant comorbidities, high-risk clinical features on presentation, and worse unadjusted outcomes. However, LAHB was not an independent predictor of in-hospital and 6-month mortality and did not carry incremental prognostic value beyond the known prognosticators in the GRACE risk models.

“…It is now well known that QT and QTc prolongation causes life-threatening arrhythmias, and it has also started to be realized that LAH is important regarding cardiac comorbidities. In studies previously conducted, LAH has been found to be associated with increased cardiac death in patients with suspicion of CAD and increased mortality in the hospital and during the 6-monthfollow up in patients with acute coronary syndrome (16,17). Hence, in patients with leprosy, in addition toQT and QTc prolongation, LAH that occurs at high rates is also an important ECG finding that should be considered, and it is highly important to monitor the QT and QTc intervals on ECG to predict arrhythmic events in these patients.…”

“…18,19) In contrast, some studies have mentioned that the development of left axis deviation in AMI is associated with a good prognosis, and this supports our result. [20][21][22] Kim, et al also demonstrated that postprocedural TIMI flow in non-ST-elevation myocardial infarction patients did not affect the clinical outcomes. 23) However, here, the patient had a potential possibility to implant a permanent pacemaker.…”

“…Zhang et al report in this issue,1 in their comparison of quality of care and outcomes of patients with left anterior hemiblock (LAHB) hospitalised with acute coronary syndrome (ACS), lower rates of coronary revascularisation and reperfusion and higher rates of in-hospital and 6-month unadjusted mortality. They suggest that the more conservative approach to ACS care is due to the coexistence of comorbidities in patients with LAHB.…”
