Clinical Characteristics and Contemporary Prognosis of Ventricular Septal Rupture Complicating Acute Myocardial Infarction: A Single-Center Experience

Abstract: Objectives: Ventricular septal rupture (VSR) is a rare but lethal complication of acute myocardial infarction (AMI). We conducted a retrospective analysis of the clinical characteristics of VSR patients and explored the risk factors for long-term mortality. Methods: In this single-center cohort study, 127 patients diagnosed with post-AMI VSR between May 2012 and April 2019 were included. Demographic, clinical, operative, and outcome data were collected. The 30-day and long-term mortality were outcomes of intere… Show more

“…Cardiogenic shock is a common complication of VSR after MI and an important risk factor for poor prognosis of VSR (7,9,10). The reported incidence rate of CS after VSR ranges from 51.7% to 71% (1,5,14,15), and the incidence of CS observed in our study was similar to that reported in previous studies. Our study also indicated the time course for the occurrence of CS after VSR.…”

“…A retrospective cohort study of 127 patients with PIVSR in the First Affiliated Hospital of Zhengzhou University in China found that the mortality of conservatively treated patients was extremely high. The 30-day mortality rate was 93.6% (73/78), and the long-term mortality rate was 96.2% (75/78); meanwhile, patients who survived early and underwent VSR repair surgery had a good long-term prognosis, which was similar for both percutaneous TCC surgery (25.8%, 8/31) and surgical repair (22.2%, 4/18) ( 14 ). Many studies have confirmed that untreated PIVSR has a high mortality rate, so timely surgical treatment is necessary.…”

“…For VSR, conservative medical treatment, percutaneous transcatheter closure (TCC) or surgical repair are usually adopted clinically. The mortality rates of patients with medical treatment alone were extremely high ( 10 ). Hence, the European Society of Cardiology (ESC) and the American College of Cardiology Foundation/American Heart Association (ACCF/AHA) guidelines for ST-segment elevation myocardial infarction (STEMI) propose surgical closure as the standard of care ( 11 , 12 ).…”
