Clinical behaviors of rare variants of papillary thyroid carcinoma are associated with survival: a population-level analysis

Abstract: ObjectiveThis study was to evaluate the risk factors of survival in patients with columnar cell variant (CCV) and encapsulated variant (ECV).Materials and methodsThe Surveillance, Epidemiology, and End Results database (1988–2013) was used to compare the characteristics of CCV and ECV with those of classic papillary thyroid carcinoma (PTC). Survival was analyzed by the Kaplan–Meier method, the log-rank test, and Cox multivariate regression. Multivariate logistic regression was used to further analyze lymph nod… Show more

“…Chen et al [54] describe nine cases (five male, four female) of CCV, with a mean age of 57 years. Bongiovanni et al published 11 specimens of FNA cytology (from three Wang et al [55] and Jiang et al [56] have both used the SEER database to demonstrate that CCV is associated with higher rates of extrathyroid extension, nodal and distant metastases, and poorer overall survival in comparison with classic PTC. Average survival in the study by Wang [56], the 10-year overall survival was 89.02% (96.98% in classic PTC; p \ 0.0001).…”

However, in the seminal study by Wenig et al [52], the patients' tumors were divided into encapsulated with minimal invasion and widely infiltrative.…”

“…All the current clinical observations suggest the WDTC patient's mortality to increase with age. The explanation of poorer prognosis causes is accompanied by the following facts: the patient group aged 60 years and above to contain a higher male part; these persons have tumors above 5 cm with more frequent metastases rate; their disease is usually attributed to the III-IV stages; it is less probable the patients to be treated by iodine isotope; most probably, these patients anamneses inform about external radiation therapy [5,16,18]. Unsatisfactory therapeutic results in elderly patients are also due to their worsened general condition (and accompanying disorders) as well as to decreased tolerance to the treatment.…”

“…Among our cohort patients, the worst results have been found for persons above 60 років. All the clinical surveys suggest currently the mortality of patients with differentiated thyroid cancer to grow with age [6,7,13]. While explaining the causes of poorer prognosis, the following facts are to be taken into account.…”
