Clinical aspects, outcome assessment, and management of ankylosing spondylitis and postenteric reactive arthritis

Abstract: The cause of ankylosing spondylitis remains unclear. Proof that this disorder is an autoimmune disease attributable to crossreactivity between bacteria and HLA-B27 is still lacking. Differences in endogenous peptide presentation by HLA-B27 subtypes might be relevant in the etiopathogenesis. Fractures of the osteoporotic spine contribute to morbidity. Spinal cord injury may occur. MR imaging enables identifying sacroiliitis earlier than plain radiography. Sweet syndrome has now been described in patients with a… Show more

“…The most common symptom is inflammatory back pain that shows the involvement of the spine and sacroiliac joints. The diagnosis of AS depends on the New York criteria: the presence of inflammatory back pain, the restriction of lumber flexion and chest expansion, and radiological findings of sacroileitis [1,2].…”

“…Differences in endogenous peptide presentation by HLA-B27 subtypes might be relevant in the disease pathogenesis. HLA associations here possibly involve arthritic peptides and molecular mimicry, but proof that ankylosing spondylitis is an autoimmune disease attributable to cross-reactivity between bacteria and HLA-B27 is still lacking (127). HLA associations also have been reported for peripheral arthropathies associated with IBD.…”

“…This form of ReA belongs to the group of spondyloarthropathies (SpA), is triggered by bacteria (which enter the body through the mucosal surfaces) from the genera Campylobacter, Chlamydia, Salmonella, Shigella, and Yersinia, and is clinically associated with oligoarthritis of the lower limbs and sometimes with urethritis and conjunctivitis (203,210).…”
