Clinical Approaches to the Diagnosis of Tick-Borne Borreliosis
Abstract: Diagnosis of Ixodes tick borreliosis (SDS) at an early stage of the disease is not perfect that creates problems in a timely diagnosis. This paper presents the results of a comparative analysis of the incidence rate of Ixodes tick borreliosis (CBI) in the Ulyanovsk region for the period 2002-2013; there is considered the pattern of mite species and the results of their research on Borrelia species (2011-2013).

Цель. Изучить частоту обнаружения антител к боррелиям (иммуноглобулинов М и G) у пациентов различного пола, возраста и места проживания в Республике Беларусь.Материалы и методы. В сети лабораторий во всех областях Республики Беларусь и г. Минске проведено методом иммуноферментного анализа (ИФА) определение иммуноглобулинов М (4007 исследований) и G (4085 исследований) к боррелиям. Были использованы тест-системы производства компании Euroimmun (Германия). Сравнение частоты выявления иммуноглобулинов М и G к боррелиям у лиц различного возраста, пола и места проживания было произведено методами непараметрической статистики (таблицы 2×2, критерий χ2).Результаты. У 19,78% обследованных были выявлены иммуноглобулины М к боррелиям. У женщин в возрасте старше 50 лет иммуноглобулины М к боррелиям обнаруживались чаще, чем у мужчин. У 13,72% обследованных регистрировались иммуноглобулины G к боррелиям. У женщин в возрасте до 18 лет иммуноглобулины G к боррелиям выявлялись чаще, чем у мужчин. Самый высокий процент положительных результатов при обнаружении IgG к боррелиям у жителей города Молодечно, в то время когда самый низкий – у жителей Жлобина (р<0,05, χ2=2,002), но необходимо отметить, что ни в одном случае эти антитела не были найдены у жителей Полоцка (но было всего 6 исследований). Самый высокий процент положительных результатов при регистрации IgМ к боррелиям выявлялся у жителей города Молодечно (31,67%), в то время когда самый низкий – у жителей Солигорска (12,1%, р<0,01, χ2=3,84).Выводы. У 19,78% обследованных были выявлены иммуноглобулины М к боррелиям. У женщин в возрасте старше 50 лет иммуноглобулины М к боррелиям обнаруживались чаще, чем у мужчин. У 13,72% обследованных регистрировались иммуноглобулины G к боррелиям. У женщин в возрасте до 18 лет иммуноглобулины G к боррелиям выявлялись чаще, чем у мужчин. Статистически значимо реже антитела IgG к боррелиям обнаруживались в Минске, чем в Гродненской области (χ2=1,988, р<0,05). Но реже всего эти антитела к боррелиям регистрировались у жителей Витебской области (5,6%). Статистически значимо реже антитела IgМ обнаруживались в Минске, чем в Брестской (χ2=2,1787, р<0,05) и в Гомельской областях (χ2=2,09, р<0,05). Таким образом, частота выявления антител к боррелиям имеет возрастные и региональные особенности. The purpose of the study. To study the frequency of detection of antibodies to borrelia (immunoglobulins M and G) among patients of different gender, age and place of residence in the Republic of Belarus.Materials and methods. In the laboratories in all regions of the Republic of Belarus and Minsk, the determination of immunoglobulins M (4007 studies) was carried out by the method of enzyme immunoassay (ELISA).) and G (4085 studies) to borrelia. Test systems manufactured by Euroimmun (Germany) were used. The frequency of detection of immunoglobulins M and G to borrelia in individuals of different age, gender and place of residence was compared using nonparametric statistics (Tables 2×2, criterion χ2).Results. Among 19.78% of the examined patients, immunoglobulins M to borrelia were detected. Among women over the age of 50, borrelia immunoglobulins M were detected more often than among men. Among 13.72% of the examined patients, immunoglobulins G to borrelia were registered. Among women under the age of 18, immunoglobulins G to borrelia were detected more often than in men. The highest percentage of positive results of IgG to borrelia was detected among the residents of the city of Molodechno, while the lowest – among the residents of Zhlobin (p<0.05, χ2=2.002), but it should be noted that there were no cases of antibodies found in residents of Polotsk (though there were only 6 studies). The highest percentage of positive results in the registration of IdM to borrelia was detected among residents of the city of Molodechno (31.67%), while the lowest – in residents of Soligorsk (12.1%, p<0.01, χ2=3.84).Conclusions. Among 19.78% of the examined patients, immunoglobulins M to borrelia were detected. Among women over the age of 50, borrelia immunoglobulins M were detected more often than in men. Among 13.72% of the examined patients, immunoglobulins G to borrelia were registered. Among women under the age of 18, immunoglobulins G to borrelia were detected more often than in men.
