Clinical applications of multi-parametric CMR in myocarditis and systemic inflammatory diseases

Abstract: Cardiac magnetic resonance (CMR) has changed the management of suspected viral myocarditis by providing a ‘positive’ diagnostic test and has lead to new insights into myocardial involvement in systemic inflammatory conditions. In this review we analyse the use of CMR tissue characterisation techniques across the available studies including T2 weighted imaging, early gadolinium enhancement, late gadolinium enhancement, Lake Louise Criteria, T2 mapping, T1 mapping and extracellular volume assessment. We also dis… Show more

“…However, even in the cases without biopsy results, the diagnosis was suggested by the combination of symptoms, laboratory testing, and decreased cardiac function, which often improved after antiviral therapy (see Table 1). While EMB is often performed in heart transplant recipients to rule out rejection, it may not be performed in non–heart transplant recipients, and there is evidence that imaging may have a role in the diagnosis of viral myocarditis 29,30 …”

“…CMR is accurate in describing ventricular morphology and functions well in characterizing cardiac tissue. CMR has a diagnostic accuracy for myocarditis of close to 80% and a negative predictive value of 70% [ 10 ]; T1- and T2-weighted images in addition to late gadolinium enhancement (LGE) improve the final diagnosis [ 11 13 ]. Nevertheless, CMR also has limitations: minimal variations can not be detected; borderline myocarditis or cytokine/humoral-mediated forms might not induce injuries to myocytes, thus no LGE can be seen; finally, the time from symptom onset to CMR performance may influence the results.…”

“…МРТ считается золотым стандартом, благодаря точности и воспроизводимости оценки показателей объемов, массы и ФВ обоих желудочков, отсутствию радиационной нагрузки. МРТ способствует верификации диагноза миокардита, васкулита, перенесенного инфаркта миокарда, нарушений микроциркуляции и перфузии миокарда, выявлению фиброза миокарда [66][67][68]. Классический протокол МРТ сердца включает в себя [69]:…”
