Clinical and surgical characteristics of infected diabetic foot ulcers in a tertiary hospital of Mexico

Abstract: Background: The objective of this study was to determine the clinical and surgical characteristics of diabetic foot ulcers in a tertiary level hospital in Mexico. Methods: We performed a longitudinal, descriptive study from July, 2012 to August, 2015 on a sample composed of 100 patients with type 2 diabetes mellitus and infected diabetic foot ulcers. We analyzed socio-demographic variables, comorbidities, characteristics of ulcers, and the applied treatment. Results: We found that the most affected areas were … Show more

“…We found that age was not a significant risk factor for amputation in patients with DFI. However, male sex and smoking, which were indicated as risk factors for amputation in diabetes‐related foot ulceration, were also found to be risk factors for amputation in DFI in our meta‐analysis. This may be because male sex and smoking have traditionally been the main factors in the development of PAD caused by atherosclerotic plaque formation.…”

“…These infection markers can be elevated in the presence of osteomyelitis, which has been shown to be a strong predictor of amputation, so these markers might indirectly predict amputation. From another perspective, the increase in acute‐phase reactants might indicate an inflammatory process triggered by an impaired immune response, deterioration of the peripheral circulation, or a high blood glucose level . Inflammation and infection may also contribute to the development of PAD, thereby allowing these inflammatory markers to predict amputation in patients with DFI.…”

“…The presence of macro vascular disease and associated infection of the ulcer increases the chances of amputation of lower limb. 3 Microbial infections of the diabetic foot ulcers are difficult to manage because of multiple factors associated in response including overall glycaemic control, associated complications like vascular disease and neuropathy. The depth of the ulcer is an important factor which decides the outcome of the diabetic foot ulcer.…”

“…It was also found that the most affected areas by DFUs were the forefoot (48%) and the plantar region (55%) of the foot. Most patients had advanced stages of DFUs, where 93% of the lesions were grades III-V, according to the Wagner classification [11]. In 2016, a report revealed evidence that hemoglobin A1c (HbA1c) <5.8% works as a beneficial factor [12].…”
